Job summary

We are looking to a practice nurse to join our growing GP practice the Alliance Teaching Practice which has a registered population of around 22,000 patients.

As a Practice Nurse, you'll deliver face-to-face services, enabling patients to obtain health information from the appropriate health care professionals. You will be providing safe and effective health assessments and treatment of patients within the establishment. You'll be willing / experienced (desirable) to work with or alongside Care Homes.

You'll be accountable for your practice including the assessment, diagnosis, treatment, and discharge or referral of patients, independent of medical intervention.

Practicing within your sphere of competence and agreed acceptable limits of practice, you'll assess patients, and order and interpret investigations, You'll be responsible for the assessment of care needs the development, implementation, and evaluation of programmes of care, without supervision, and teaching other nursing and non-nursing staff.

Working as part of the multi-disciplinary team in providing quality, evidence-based health care to meet the immediate needs of the patient, you'll take an active role within the cross-disciplinary, multi-skilled team to ensure service quality in line with corporate goals and strategy; to assist in the implementation of the wellbeing approach which has a positive impact on the health economy.

Main duties of the job

Utilising your skills in managing patients within a primary care setting and dealing with physical health concerns, mental health issues and minor illnesses, you'll be an integral member of the medical team, conducting clinics independently, completing administrative tasks (prescriptions, referrals, pathology results) and supporting our Doctors and reception processes.

Leading by example, you'll share your experience and knowledge by being involved in the educational and professional development of the nursing team through clinical supervision sessions both on the wings and formal teaching opportunities.

Summary of Responsibilities

The Alliance Teaching Practice operates from 4 sites, including Foleshill, Stoke Aldermoor, City of Coventry Health Centre and Broad Lane Surgeries.

The postholder will be expected to undertake the following as part of their role:

Work independently to provide excellent practice nursing care directly to patients

Participate in the delivery of routine surveillance and screening.

Participate in the management of long-term conditions, contraceptive services, wound care, immunisations and vaccinations.

Participate in audit and research to ensure high quality care.

Support the development of innovative and high-quality nursing practice, offering advice on service improvements based upon your experience and knowledge of the needs of patients

Main Duties

Clinical:

To work within the multidisciplinary team to meet common goals.

To participate in long term condition management for patients according to explicit protocols

To provide family planning advice

To administer IM Contraceptive injections as per protocol

To administer childhood immunisations according to the national vaccination schedule

Monitoring of ongoing treatments.

To provide health education and cancer screening advice.

Perform cervical cytology screening.

To provide opportunistic screening and advice on mens health.

To assist with planning and evaluating patient care.

Clinical Measurements such as blood pressure recording, blood glucose monitoring and ECGs

The administration of subcutaneous and intramuscular injections

Undertake wound care and appropriate management of wounds.

To provide Health Promotion in areas such as weight, smoking and alcohol.

Teaching patients and their family self-care where appropriate.

Assisting and or chaperoning the general practitioner.

To work consistently within your own professional standards, maintaining an awareness of evidence based and current best practice.

To ensure appropriate registration with your own professional body.

To participate fully in the annual appraisal process and maintain own personal professional development.

To participate in clinical supervision

To advise, support and contribute to the education of other members of the multidisciplinary team.

Maintain cleanliness and hygiene to infection control standards

To participate in the maintenance of medical equipment, ensuring appropriate interim checks are carried out and equipment is appropriately calibrated, and faults are reported in line with policy.

To support the maintenance of appropriate stock items in fridges and clinical areas and to adhere to cold chain policies

Record Maintenance:

Keep contemporaneous clinical records using the computer system (paper records where appropriate), recording clinical history and examination findings, interventions and advice given.

Ensure documentation is in line with NMC guidance

Stock Control & Equipment:

Have shared responsibility for the Treatment room stock and report any stock shortages to the service lead

Maintain nursing drugs and equipment reporting any shortages/failures to the service lead
