Solutions4Health are delighted to be offering the role of Community/Home Care Stop Smoking Specialist at the Royal Borough of Windsor & Maidenhead. This is a full-time job with a salary of £30,000 per annum.

Role Summary 

Solutions4Health is a leading organisation committed to tackling health inequalities, and we are seeking an enthusiastic Community/Home Care Stop Smoking Specialist to support clients in quitting smoking. In this role, you will deliver client-led smoking cessation support through one-to-one, drop-in, or group sessions, tailoring treatment plans to individual needs. As part of our outreach efforts, you will also provide home visits for those unable to attend clinics, ensuring accessible and personalised support.

We are looking for a dedicated, organised, and proactive individual with a solutions-focused mindset and strong relationship-building skills. You should be an effective communicator, able to collaborate with stakeholders, manage workloads efficiently, and develop innovative community engagement strategies. Experience working with vulnerable groups who may face barriers to accessing healthcare is essential.

Main duties of the job

Main Duties and Responsibilities

Explain to clients the principles of Stop Smoking Services so that they can make an informed choice to stop smoking.

Conducting community/home visits of smokers and providing specialist support.

An outgoing personality with the ability to engage confidently and effectively with a vast spectrum of individuals and interest groups.

Ability to build, strengthen and maintain partnerships with care homes and refugee sites.

Motivate and empower clients to make behaviour changes.

Ability to work independently and as part of a team.

Use your excellent communication skills to educate, influence and motivate others.

Commitment to addressing healthcare inequalities and successfully reaching out to vulnerable groups within the community, which could include people recovering from addictions, young parents, areas of deprivation and more. 

Devise and deliver a client-led treatment plan in concordance with national and local guidelines. 

The completion of accurate data to comply with NICE guidelines, and a comprehensive account of weekly interventions.

Weekly CO monitoring on each visit

Plan and organise workload, this will include the management of a caseload.
