JOB SUMMARY

This may involve, family health assessments in line with the 5 mandated reviews, giving advice and support, and signposting to other agencies.
Key Responsibilities:
• Work in partnership with individuals, families, and communities to assess, plan, implement, and evaluate programmes of care for individuals and families.
• To plan and implement programmes that promote and protect health such as screening programmes, health promotion campaigns, immunisations, planning, and delivering integrated services across agencies.
• To provide evidence-based health promotion advice to families on various issues such as coping with a crying baby; healthy sleep patterns and safer sleeping; infant feeding including breastfeeding; growth monitoring what to do if your child is unwell; introducing family foods and solids; behaviour and understanding your baby/child; immunisations; emotional wellbeing; speech and communication; developmental concerns.
• Take responsibility for ensuring engagement with service users who are receiving a service virtually and ensure this is done at a high standard and in line with S4H policies and procedures.
• To work as part of a corporate 0-19 (25) team • To identify health inequalities and take appropriate action to address these, by working collaboratively with health, statutory and voluntary agencies to address the wider determinants of health (i.e.) housing, transport, and social exclusion.
• To work with communities and services users to develop and design services to meet identified needs and support family health assessments such as mental health needs.
• To work in accordance with local and national child protection policies to identify and assess situations where children are subjected to abuse and children who are looked after. This may include attending strategy meetings, case conferences, core group meetings, and report writing.
• To identify, assess, and plan a programme of care where there are children with disabilities and special needs.
• To advise and refer to the Specialist Health Visitor for Vulnerable Families (i.e. refugees and asylum seekers) of services available through the NHS and other statutory and voluntary agencies. Staff is expected to be flexible and adaptable covering colleagues, supporting outreach activities, and service provision during periods of sickness, annual leave, training, and any other absence.
Service delivery and clinical governance:
• To deliver high-quality care by using valid research and evidence-based practice, which will maintain and or/improve the health and well-being of individuals, families, and communities
• To contribute to clinical governance in practice, managing quality assurance, including audits. To have an active role in implementing change and improving practice.
• Participate in clinical governance/risk management activities to ensure the highest quality of the practice is maintained.
• To work within the Nursing & Midwifery Council’s (NMC) framework for practice. All Specialist Community Public Health Nurses should have knowledge and work in accordance with S4H policies and guidelines including clinical supervision. It is the responsibility of each staff member to update their mandatory training as required.
Professionalism:
• To use effective organizational skills and expertise to effectively support and manage a multi-disciplinary team in line with the Healthy Child Programme. This team will include nursery nurses, and other nursing bands as appropriate.
• To manage clinical activities within their caseload/workload and effectively prioritise these activities, to ensure that electronic records are maintained in accordance with S4H policy.
• To ensure that delegation of work is appropriate and provide/ participate in clinical supervision.
• To participate in and promote personal development reviews (PDR) and appraisals.
• Participate in audits and review of project activities and their performance when needed.
• Attend induction and mandatory training to support Virtual Ward activities, implementation, and evaluation.
• To adhere to Virtual Ward policies and procedures as applicable
