Job summary
As a key member of the leadership team, the Team Leader will be responsible for the day-to-day leadership and management of a designated team of health visitors, Community Staff Nurses, Nursery Nurses and other members of the multidisciplinary team. Leading, influencing and delivering a comprehensive public health nursing service in line with the Healthy Child Programme and local service specifications that meets the public health needs of local communities.
The Team Leader will be required to support the Locality Lead, ensuring key performance indicators are met and lead on the delivering of the Healthy Child Programme. The role will involve working within a Locality managing Health Visitors and a skill mixed team and there will be a reduced caseload requirement for this post.
The post holder will work to the standards outlined in Working Together to Safeguard Children (2018), along with improving health and life chances for children and families and safeguarding those at risk. To deliver the role in accordance with S4H values and behaviours.
Main duties of the job
JOB PURPOSE
To provide clinical and professional leadership and manage 0-19 public health nursing health visiting and school nursing teams, ensuring the delivery of a comprehensive and high-quality needs led services.
To plan, organise and evaluate an innovative public health service, and where appropriate delegate to other members of the team to ensure delivery of a high-quality service.
To provide a skilled, effective and evidence based public health service to clients in a variety of community settings.
To work in collaboration with other healthcare and service providers to ensure seamless delivery of services for clients and communities.
To collaborate with team colleagues in order to deliver the Quality Strategy objectives.
Plan, organise, evaluate and where appropriate, delegate to other members of the 0-19 team to ensure delivery of a high-quality service.
MAIN DUTIES AND RESPONSIBILITIES
Clinical duties
To act as a clinical resource and provide clinical management and professional leadership for the 0-19 (25) team, which supports and facilitates the development of the public health service
To provide a skilled, effective and evidence-based service to clients in a variety of community settings to include school, home, GP, children’s Centre and community clinics.
To ensure a continuous high quality of service delivery by leading, modelling and promoting an evidence-based approach using relevant assessments tools, NICE Guidance, National Service Frameworks, NMC standards and the Maternal Early Childhood Sustained Home visiting (MECSH) programme.
To monitor the quality-of-service provision, reviewing key performance indicator reports and identifying areas for service and professional development and facilitate their implementation.
To lead, manage and supervise team members to ensure successful delivery of clinical, restorative and safeguarding supervision.
Successfully lead and deliver on the 6 High Impact Areas of the Healthy Child Programme.
Support colleagues to appropriately refer, in partnership with families and other professionals, children requiring secondary assessments.
Any clinical responsibilities provided must be delivered within the professional area of expertise i.e. health visiting or school nursing
Lead and provide evidence-based practice relating to safeguarding children supporting and enabling colleagues working with vulnerable families, Child in Need, children Looked After and children subject to Child Protection Plan
To have an overview of the profiled health needs for the local community ensuring that the 0-19 service meets the needs of the local population, that resources are utilised effectively, and any service gaps identified and addressed.
To support the identification, prioritisation and implementation of health assessments, programmes of care, to ensure the delivery of the healthy child programme
To work with the team with the planning, organising and coordinating of health clinics or health promotion groups as appropriate to the local population
To have an overview of the children known to be at risk and / or in need within the team. To liaise with the safeguarding team as necessary and GPs to share information as appropriate according to the Solutions 4 Health policies
To ensure that team members are attending safeguarding supervision. To actively promote an approach of professional curiosity within the team and awareness of contextual safeguarding
2. Quality and performance
To monitor standards and performance of the team, in line with quality governance and S4H objectives. Manage objectives set locally, as a team and individually
To contribute to the undertaking and reporting of performance measures, alerting Clinical Lead of any shortfalls
Champion the use of SystmOne electronic record keeping system and engage in and promote the use of electronic diaries to accurately reflect the working day.
Provide gap analysis and action plans to resolve non-achievements of standards and key performance indicators, alerting senior management to any risk of non-achievement
3. Management and leadership
To establish and maintain a good working relationship with colleagues, the wider Locality health care team and other health, education and social care providers, so as to provide a comprehensive service to clients
Have full leadership and management responsibility in the locality for Health Visitors, Staff Nurses, Nursery Nurses and other staff providing clinical caseload management support, supervision and review.
Enable the effective allocation of caseload work within the team and locality with additional reference to risk assessment, safety, mobile and lone working policies.
To lead and contribute to the delivery of the HCP and identify and contribute to further multi-disciplinary assessment of children with additional needs including children with special educational needs and disability (SEND).
To provide facilitative and supportive leadership and management to the public health team members
To initiate and where appropriate take the lead responsibility for service development activities.
Together with colleagues across S4H identify and manage the levels of staffing required to meet service needs utilising established resources
