Job summary

The Asthma Nurse will be the clinical expert and professional resource to the multi-professional team, patients, and carers within their Speciality Service.

This is a new service that is being delivered by practices within the Dacorum locality on behalf of Delta PCN.

As a Band 6 Respiratory Nurse you will undertake all aspects of the role using advanced clinical assessment skills and respiratory evidence-based knowledge.

This will be achieved through the core dimensions of the role, clinical practice, and acting as the patient advocate and through audit and education.

The PCN values the contribution of specialist nurses and supports the post holder contributing to the development of nursing aligned to objectives and goals.

We are looking to appoint highly a motivated Respiratory Nurse who has a passion for delivering safe, high-quality care to patients in the primary care setting. The successful candidate would gain a wealth of knowledge and experience surrounding respiratory medicine.

Main duties of the job

Develop a high level of clinical decision making, monitor and improve standards of care, working within their advanced scope of professional practice.

Clinical care by managing a patient caseload providing expert assessment, planning and evaluation.

Manage nurse-led clinics (within Speciality/service).

Contribute towards a multi-professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements.

Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery.

To support, assist and encourage the multi-disciplinary team in resolving patient problems, by the provision of new and innovative models of case management.

About us

Primary Care Networks (PCNs), supported by Dacorum Healthcare Providers Limited (DHPL), are developing new and exciting additional roles under the 2020 scheme to support GPs and clinical teams at enhancing patient services in the local community.

PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY

Clinical

You will undertake specialist respiratory assessment of patients nursing needs, plan, implement and evaluate care delivery using clinical knowledge and clinical skills.

You will ensure all documentation and communication is maintained to a high standard.

  • You will liaise with members of the multi-disciplinary team and refer to other services and health care professionals where appropriate ensuring holistic care.
  • Manage a patient caseload of those requiring long term support for asthma providing expert assessment, planning and evaluation.
  • Contribute towards a multi-professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs.
  • Establish and maintain effective communication with patients and carers / relatives.
  • Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery.
  • Recognise changes in patients condition that require the intervention of others and refer on as appropriate.
  • When appropriate you will be expected to undertake a non-medical supplementary and independent prescribing qualification in order to prescribe within area of speciality for your patients.

Leadership

  • Act as a role model and specialist nurse providing specialist, clinical knowledge to clinical colleagues through MDT meetings for patients and carers / relatives.
  • Undertake clinical supervision of nursing colleagues on an individual or group basis.
  • Act as a resource for health care professionals by being visible, available, and accessible for support and advice in relation to the management of patients in the speciality.
  • Participate in the development of services and assist to implement change, linked to directorate business plans.
  • Participate in the development of strategies for delivering effective specialist care within a changing environment that improves the quality of care and health outcomes in the specialist area.
  • Monitor health, safety and security of self and others and promote best practice in the clinical area.
  • Participate in ensuring systems are in place for risk assessment and minimisation including promoting no blame cultures in order that near misses and incidents are reported.
