SOCIABLE HOURS - MONDAY TO FRIDAY (9AM TO 5PM)
The first round of interviews will take place via Teams Meeting within two weeks of the closing date.
The Paramedic Practitioner must have at least 5 years' work experience as a Paramedic and will work autonomously both within the community and within the PCN and Practice based multi-disciplinary team. They will use their enhanced clinical assessment and treatment skills to provide chronic care management or first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or injury, abdominal pains, chest pains, headache and mental health crisis.
The practitioner will have direct and immediate access when needed to a GP/Doctor and work under supervision of the GP with which the patient is registered.

Primary Care Networks (PCNs), supported by Dacorum Healthcare Providers Limited

the collection of pathological specimens including intravenous blood samples,
swabs etc.
Support
the delivery of anticipatory care plans as this process develops (e.g.
monitoring blood pressure and diabetes risk of elderly patients living in
sheltered housing).
Provide
an alternative model to urgent and same day GP home visit.
Maintain
accurate and contemporaneous health records appropriate to the consultation,
ensuring accurate completion of all necessary documentation associated with
patient healthcare and registration with the practice.
Other Duties:
Work
within the policies of scheme of the practices and PCN.
Promote
client involvement in the management of the service.
Participate
in regular supervision sessions and appraisals with your line manager.
Attend
training and development activities as identified and participate in meetings
as required.
Maintain
administration systems and workspace used in a clean and tidy condition.

flexibly to meet the needs of clients.
To
undertake any other duties appropriate to the grade and purpose of the job as
may be agreed by the post holder.
For
those patients at risk of hospital admission, for example, in a care home or
with complex needs they have a care plan which is communicated to the out of
hours GP service.
On-going training / study leave will

- BSc in a training programme approved by the College of Paramedics
- Health & Care Professions Council (HCPC) registration
- able to operate at an advanced level of clinical practice
- has completed the 2 year Consolidation of Learning period as a newly qualified paramedic
- has a further three years experience as a band 6 (or equivalent) paramedic
- Framework for Higher Education Qualification (FHEQ) Level 7 or Scottish Credit and Qualifications Framework (SCOF) Level 11
- ability to write comprehensive clinical notes, implement and evaluate care plans
- ability to evidence a sound understanding of the NHS principles and values
