Job summary

We are a leading edge Primary Care Organisation who has led the development of many innovations including seven-day services delivered from four local host sites in the City. Our size and excellent relationship with Primary Care enables us to deliver large scale and impactful improvements for our patients with a clear focus on ensuring we tailor services for all of our local populations served by our Primary Care Networks.

PCM believe that the Federation and PCNs have a shared purpose of developing high quality local services for local populations while simultaneously building resilience for General Practice. CMN want to support our PCNs by releasing the administrative burden so that they can focus on making clinical improvements to local services. Our approach can ensure that we reduce duplication of effort and resource for those at scale projects that the PCNs are beginning to prioritise.

Main duties of the job

As the Primary Care Networks (PCNs) Network Liaison Manager, you will report to the Operations Manager at CMN, to support the successful operation of the PCNs supported by the Federation.

You will provide high quality administration and project support to the Primary Care Networks including the PCN Clinical Directors and the CMN Team supporting PCNs who will, in turn, support you in your role. You will be supported by the PCN Finance Manager and CMN's Service Manager who are both part of the Federation team involved in supporting PCNs.

Typical tasks will include project planning, tracking delivery of projects, engaging with PCNs and associated stakeholders to maintain support for projects. The post holder will ensure a high level of ongoing communication and engagement with all PCNs, to ensure we are aware of their priorities including emergent themes. As and when required you will need to support the Operations Manager with the administration tasks associated with these projects.

Job responsibilities

The post holder will be expected to:

  • Provide high quality management support to PCNs to enable delivery of their objectives.
  • Act as a liaison point between PCN Clinical Directors, the Practice Managers supporting the PCN and the Operations Manager who links directly into the Board of Directors.
  • Use knowledge of local or national strategies and frameworks to further develop the work of the PCNs, seeking opportunities to gain additional funding to enhance the scope and pace of development
  • Lead identification of information requirements that will support delivery of PCN projects.
  • Lead collation activities and analysis of information that will support delivery of PCN projects.
  • Lead work with Clinical Directors to support the successful operation of Primary Care networks, including supporting with engagement activities and working closely with MLCOs representatives around community cohesion and services development.
  • Lead the work with the Clinical Directors to track local delivery and report on local progress to CMN.
  • Support the Federation Operations Manager in writing bids and contract management activities including outcomes monitoring and gathering case studies
  • Support the Finance Manager with any cost/benefit analysis of PCN work.
  • Represent the Federation at PCN internal and external meetings.
  • Liaise with members of the Manchester Local Care Organisation (MLCO) including their Primary Care Locality Lead to ensure workstreams are aligned and to avoid duplication.
  • Liaise with Neighbourhood Clinical and non-clinical Leads and to attend neighbourhood meetings where appropriate to maintain up to date knowledge and experience within the role
  • Produce reports on strategic/operational development to relevant parties to promote the work of the PCN and to demonstrate for progress against objectives.
  • The post holder will be required to maintain constructive relationships with a broad range of internal and external stakeholders including but not limited to the Federation, GP practices, MLCO, Community services, Secondary/Acute Care, Mental Health and Third Sector providers (CVS).
  • Participate and on occasion take the lead in relevant internal and external working groups to support project delivery.
  • Work closely with the Operations Manager, Board of Directors and PCN Clinical Directors to deliver PCN objectives.
  • Lead on providing analysis and associated briefings and reports to internal and external stakeholders including sharing these with PCN CDs and at PCN monthly meetings.
  • Lead on work with members of internal and external IT and information teams to implement data collection systems that will provide accurate and timely data.
