Main duties of the job

Job Requirements:

You will be a qualified Nurse or due to qualify RMN/RNLD. Main Role Purpose

To work according to the NMC Code of Professional Conduct and other professional and legislative guidelines as a named nurse for a number of patients, taking responsibility for:

Protecting, supporting and improving the health of individual patients

Assessment of holistic care needs for patients.

Development, implementation and evaluation of care and treatment in collaboration with the individual patient.

Undertake active participation in effective discharge planning

To ensure the optimum quality of care for patients at all times.

To support the Senior Staff Nurses, Managers and senior clinical staff in all aspects of ensuring high standards of Care delivery and compliance.

To work proactively within the wider multidisciplinary team, fostering team work and ensuring the holistic assessment and treatment needs of the patients are being met.

To assist in and support staff to achieve high quality care through innovative and evidence based practice.

To act as an ambassador for the service and organisation through all interactions upholding the reputation and professionalism of the Team.

Main Duties/Responsibilities

Act as a Named Nurse for identified patient(s)-ensuring all aspects of Care Planning are implemented and reviewed according to relevant protocols.

Promote and engage patients actively in involvement in their care.

Ensure all Care plans have risk assessments that are relevant, clearly identified and risk management plans are clear and consistent.

Ensure that all standards of ward procedure are implemented to the highest standard.

Ensure policies for the safe storage, custody and safe administration of medicines are carried out to the highest standard.

Assist the management and multidisciplinary teams in ensuring that all Clinical documentation/procedures comply with all applicable legislation.

Undertake to provide clear concise, accurate and objective documentation in the form of:

o Reports

o Daily recording entries

o Measurement and result recording

o Medicine administration records

o Security forms

o Financial records

o Other documentation related to job role

Attend and take part in Ward Rounds, CPA , MHRT as required.

Ensure correct procedures take place for the Admission and Discharge of Patients.

Assist in directing, supporting and training Junior staff in the management of work flows.

The role can be physically demanding and there is a requirement for all ward-based staff to be able to undertake physical intervention training; and to be able to utilise these skills post training.

Business

Assist in ensuring all relevant KPIs and Outcome measures are met, relevant to the Ward

Maximise the use of resources and reduce waste/usage where possible

Human Resources

Maintain a professional position at all times.

Maintain a personal professional portfolio and ensure that you implement evidence based clinical practice.

Undertake and promote active engagement in clinical supervision, carryout clinical supervision for Junior Staff as required.

Ensure Support staff are only asked to perform tasks that are within their area of responsibility and levels of competence.

Health and Safety

Attend all Health and Safety updates and training required as part of hospital statutory training.

Assist in the maintenance of physical security and safety through regular monitoring and checks.

Actively undertake responsibility for the Health, Safety and Welfare of him/herself and other employees in accordance with legislation and specific Policies and Procedures.

General
