Job summary

Cheswold Park Hospital are looking for an enthusiastic and dedicated Social Worker to join the team at our forensic hospital. We have steady expansion plans and we have recently added a further MSU male ward. Our primary focus is to support patients through the discharge process and minimise lengths of stay, through discharge planning with active patient and carer involvement.

Job Requirements

You will have a relevant professional qualification in social work and be Social Work England (or equivalent) registered. Ideally you will have experience within a similar environment whilst having a real passion for discharge planning. An interest in and commitment to working with adults with various mental health issues is essential. Days of work are open for discussion, but will need to meet the needs of your allocated patients.

Main duties of the job

Key responsibilities

  • Working in partnership with all internal and external professionals involved in assessment and service provision, through effective communication and consultation, to ensure a co-ordinated response to patients and carers, with the overall focus always being on discharge planning.
  • Working with carers and family members, working with home area health and social care services and assessing appropriate community resources.
  • Planning for discharge and recording 117 aftercare arrangements.
  • Provide and present relevant reports for CPA, Hearings & Tribunal panels.
  • Ensure all reports and documentation are recorded accurately and concisely in line with The Code of Ethics (General Social Work Council) and The Mental Health Act Code of Practice. Contribute to MDT discussion around risk management and care plans in accordance with own professional standards.
  • Attend Clinical Review meetings, Ward rounds and present evidence of social work intervention.

Responsible for: Social work assistants and social work students.

Summary of Purpose

To support the clinical team in managing the health and social care needs of the patient population. Ensuring high standards of Care delivery and compliance.

To assist in and support staff to achieve high quality care through innovative and evidence based practice.

To ensure that you uphold the reputation and professionalism of the Team.

Main Duties/Responsibilities

Act as a Named Social Worker for identified patient(s) - ensuring all aspects of Care Planning are implemented and reviewed according to relevant protocols.

Ensure all reports and documentation is recorded accurately and concisely in line with The Code of Ethics (General Social Work Council) and The metal Health Act Code of Practice. Contribute to MDT discussion around risk management and care plans in accordance with own professional standards.

Ensure that all standards of hospital procedure are implemented to the highest standard.
