Job responsibilities
Supportive Care Deputy Team Lead:
Providing
therapeutic counselling and group support to hospice clients, adults and
children, who are experiencing the emotional and spiritual impact of life-threatening
illness, death and bereavement.
Being
an expert resource within the team and wider hospice MDT
Providing
direct line management to designated members of the Supportive Care team
Working
in collaboration with colleagues across the hospice to plan, support and
deliver care that is person centred and coordinated.
Triaging
referrals to the Supportive Care Team, assessing individual needs and agreeing
the appropriate service offered by the Supportive Care department or referring
to other external agencies for appropriate therapeutic treatment
Providing
individual counselling and opportunities for creative expression, offering a
safe, boundaried relationship within which a person can experience the freedom
and relief of being listened to with compassion and without judgement.
Facilitating
therapeutic groups where group members are supported in exploring any troubling
unknown territory and sharing issues and concerns they may not feel able to
speak freely of at home.
Line
managing specific members of the Supportive Care Team, substantive and
volunteer, and providing supervision for designated Supportive Care Team members
Taking
responsibility in the absence of the Team Lead, for the operational leadership
of the service
Taking responsibility
for specific, delegated aspects of leadership within the Supportive Care team
and support the Team Lead in the effective and efficient use of resources
Contributing
to the governance of our services through compliance with and involvement in
reviewing and producing policies, procedures, protocols and guidelines relating
to clinical care and relevant health and safety issues.
Ensuring
that services are made available to clients / service users both on an in-patient
and out-patient basis at Deer Park, The Long House and other community
locations, including where necessary, clients own homes
In-putting
patient/client data into the patients electronic record within the agreed time
standard.
Maintaining
patient activity statistics and compiling reports as requested.
Ensuring
that confidentiality of information is maintained at all times.
Participating
in effective team, multidisciplinary and multi-agency working to facilitate
seamless, coordinated care
Working
with other departments and individuals within the hospice to ensure cohesive
working, sharing of information, development of practice and integrated working
Participating
in and contributing to training programmes provided by the hospice.
Leading
on projects, improvements to service delivery and practice that will enable the
hospices Supportive Care team to develop and meet future needs of the hospice.
Participating
in the processes of audit, clinical governance and the setting and monitoring
of standards.
Creating
opportunities for and maximising the contribution of volunteers in the delivery
of the Supportive Care service
Ensuring
that you follow good infection control practice at all times and that you are
familiar with infection control policies, procedures, and guidance relevant to
your area of work.
Understanding
your role in the safeguarding of adults and children by being able to identify
risks, and recognise signs of harm, abuse or neglect including how to report
concerns and take immediate action to protect a person at risk of harm or
abuse.
Supportive Care Counsellor:
Providing

Assessing
and helping hospice clients explore which therapeutic services offered by the
Supportive Care Department would be appropriate and of benefit to them and
allocate to the appropriate Supportive Care staff and/or signpost them to other
agencies where necessary.
Providing
counselling, offering a safe, boundaried relationship within which a person can
experience the freedom and relief of being listened to with compassion and without
judgement.
Facilitate
therapeutic and informal groups where group members are supported in exploring
any troubling unknown territory and sharing issues and concerns they may not
feel able to speak freely of elsewhere.
Prioritising
and managing a caseload.
Providing
a service that is flexible and responsive to client need.
Engaging
in co-operative relationships with all hospice staff and volunteers and
external agencies and organisations.
Keeping
accurate client records on the hospice computer system and compile reports as
required.
Participating
in the monitoring, review and auditing of the therapeutic services provided.
Attending
to personal and professional growth and development.
Actively
participating in supervision.
Complying
with all standards and guidelines set by the British Association of Counselling
and Psychotherapy.
Ensuring
