The Carers Hub Officer plays a pivotal role in supporting unpaid carers by acting as a first point of contact, providing information, guidance, and access to services at local ABHB hospitals. The role involves managing day-to-day operations of the carers hub, ensuring that carers receive timely, personalised support, and facilitating access to a range of resources aimed at improving their wellbeing and sustaining their caregiving relationships.

Working 10am - 2pm 4 days per week

Main duties of the job

  • Act as the primary point of contact for carers via ward visits, and in-person enquiries at the Carers desk.
  • Provide accurate information and referrals to appropriate services, including, health, financial, and emotional support.
  • Assess carers' needs through structured intake interviews.
  • Develop and maintain individual care plans in collaboration with carers.
  • Monitor progress and follow up regularly to ensure carers needs are met.
  • Maintain up-to-date leaflet supplies of local support services, community resources, and partner organisations.
  • Organise and promote carers seasonal events.
  • Completion and submission of daily evaluation log
  • Keep detailed records of interactions and service outcomes.
  • Prepare reports and data for internal review and external funding bodies.
  • Assist in the development and dissemination of informational materials.

Key Skills &Qualifications:

  • Previous experience in a support, case management, or community outreach role.
  • Excellent communication and interpersonal skills.
  • Strong organisational skills and attention to detail.
  • Empathy and understanding of the challenges faced by unpaid carers.
  • Ability to work both independently and as part of a team.
  • Proficiency in standard office software and databases.
  • Relevant qualifications in social care, community development, or a related field are desirable.

Hospice of the Valleys is the specialist provider of palliative care services in Blaenau Gwent. Our teams supported over 600 patients last year in their preferred place of care, offering nursing, social work, bereavement support, complementary therapies, welfare rights advice and dedicated support for carers through our Carers Hub.
