Main duties of the job
We have a great opportunity for an experienced respiratory nurse to join our team delivering PR at local venues across East and South East Staffordshire delivering high quality Pulmonary Rehabilitation (PR).
As well as seeing patients face to face - onsite in community-based venues, you will have the opportunity to test your skills to expand the access for people requiring PR.
As part of your role you will be responsible in supporting, organising and leading the team with group and 1:1 exercise and educational sessions for patients. You will complete in-depth assessments and build exercise plans to optimise the patients physical capacity within the limitations of their respiratory diagnosis, provide self-management plans and facilitate behavioural change for patients to better manage their respiratory conditions and enhance their quality of life.

Principal Responsibilities
- Registered with the HCPC and Chartered Society of Physiotherapy (CSP), adhering to professional guidelines, clinical governance, and maintaining personal development through CPD and clinical supervision.
- Assess, plan, and deliver face-to-face, digital, and home-based Pulmonary Rehabilitation (PR) programmes across allocated locations, using platforms like CliniTouch and telephone consultations.
- Conduct thorough clinical assessments, including subjective and objective measures, risk assessments, and evaluation of clinical outcomes (e.g., MCID, PROMs), ensuring appropriate exercise prescription.
- Provide specialist, evidence-based care, using clinical reasoning to tailor treatment plans, and advising patients on respiratory self-management and lifestyle modifications.
- Communicate effectively with patients, carers, and multidisciplinary teams, including delivering complex or sensitive information, and using motivational strategies to promote engagement and self-management.
- Ensure informed consent, assess patient capacity, and respect confidentiality, privacy, and dignity always, working within legal and ethical frameworks.
- Keep accurate, concurrent patient records in line with legal and organisational standards and contribute to audit and evaluation of all PR service types.
- Promote health and wellbeing, deliver education tailored to individual respiratory needs, and involve families/carers as appropriate in the rehabilitation process.
- Work autonomously and collaboratively, prioritising workload effectively, contributing to service outcomes, and supporting quality standards such as those set by the CQC.
