Main duties of the job

Our approach is focused on giving brilliant access to the right care for patients when they need it, empowering patients to self-manage, and delivering exceptional patient-centred care all with a sensible workload for our clinicians.

You will be working within a skilled and friendly team to deliver telephone, online and face-to-face consultations. Your team will include a range of clinical expertise from GPs, nurses, pharmacists, physicians, healthcare assistants and additional PCN roles. There is a strong lead team of Practice Managers and Clinical Leads supporting throughout your journey, additional back office support services and patient services teams across the practices to assist.

You will have 1:1 support and protected clinical supervision time with the clinical lead for the practice. Job description

Job responsibilities

Clinical:

To provide clinical care to patients as described in the job plan, this will include consultation sessions, telephone triage, e-appointments, telephone consultations, correspondence, results, medication reviews and prescription signing and other clinical administration, safeguarding lead, medicals, reports, home visits, practice meetings and on-call (excluding out of hours).

To adhere to the highest personal and professional standards as set out by the GMC and to put individual patient care as the priority.

To make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other healthcare professionals within the organisation.

To assess the health care needs of patients with undifferentiated and undiagnosed problems.

To screen patients for disease risk factors and early signs of illness.

To develop care plans for health, in consultation with patients and in line with current practices disease management protocols.

To provide counselling and health education.

To admit or discharge patients to and from the caseload and refer to other care providers as appropriate.

To record clear and contemporaneous consultation notes in SystmOne/EMIS to agreed standards.

To collect data for audit purposes.

To compile and issue computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible).

To prescribe in accordance with the current prescribing formulary (or generically) whenever this is clinically appropriate.

To communicate effectively with patients, carers and other members of the primary care team, recognising peoples needs for alternative methods of communication and respond accordingly.

Operations

To work with the Practice Manager and practice Clinical Lead to support the development of the practice including list growth, patient satisfaction levels, QOF, enhanced services, KPI targets, resource profile and cost management.

To work closely with the practice primary health care team in delivering clinical care, including attendance at practice team meetings.

Personal Professional Development:

To ensure on-going CPD to keep abreast of new clinical developments and maintain an appreciation of the commitments and requirements of the wider clinical workforce.

To ensure that personal mandatory and organisationally required training is up to date.

To participate in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development.

To take responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.

To ensure effective engagement with, and meet professional development requirements for, NHSE Annual Appraisal and Revalidation, PDP production and GMC revalidation.

To attend training and events organised by the Spirit or other agencies where appropriate.

Quality:
