We are seeking a talented, qualified ANP with a focus on our care homes and most frail patients.

We are on a mission to deliver a service where patients quickly and easily receive the right care from the right person, at the right time, in the right place, and this dedicated support role will ensure our care home and most frail patients receive the care they need.

Main duties of the job

The Role

Our approach is focused on delivering brilliant care with a particular focus on our most frail patients and patients within care homes linked to our Leicestershire practices.

You will lead the service deliveringtelephone, online and face-to-face consultations across Leicestershire.

We work hard to develop our practices to make sure patients get the very best care, and that we have the very best people working in them!

Job responsibilities

Clinical

To lead on care home patients and frailty patients delivering a service over the telephone, digitally and face-to-face.

Take and record a patients history including medical, family, and social history, talking to the patient, relatives, or other carer as appropriate. There may be significant barrier to understanding and acceptance due to sensitive or contentious information given and received.

Conducts a comprehensive clinical examination of the patient including a physical examination of all systems and a mental health assessment as required.

To recognise the need to initiate and where appropriate interpret relevant diagnostic investigations.

To recognise the need for and initiate where appropriate advice from or referral to relevant specialisms.

To have the ability to make clear decisions with confidence and communicate these effectively, whilst applying high level decision-making and assessment skills to formulate appropriate differential diagnoses based on synthesis of clinical findings

Formulates an action plan for the treatment of the patient, synthesising clinical information based on the patients presentation, history, clinical assessment and findings from relevant investigations, using appropriate evidence-based practice

Prescribe treatment including medications as a Non-medical Prescriber based on a sound knowledge of pharmacology, whilst considering and Implementing non-pharmacological related interventions/therapies, dependent on situation and technical requirements of care

To maintain appropriate clinical records, ensuring that documentation clearly demonstrates the NMC Guidelines for Records and Record keeping.

Independent Prescriber.

Actively engage in MDT meetings.

To work within the parameters of his/her skills and knowledge, to recognise the limitations of ones own practice and to work within the bounds of ones training and capability according to the NMC Code of Conduct.

To maintain an awareness of developments in clinical practice.

Clinical Governance

To participate in the clinical governance requirements within the Spirit Healthcare Ltd framework.

To practice with the parameters of company policies, protocols and guidelines.

To participate in quality improvement within the organisation.

To assess and evaluate outcomes of own practice, service intervention and treatment and identify any short falls in service reporting to relevant managers.

To ensure the requirements of Health and Safety and Information Governance legislation appropriate EU Directives and COSHH regulations are maintained.

To participate in research and development activities of Spirit Healthcare Ltd.

Innovate, develop, and lead evidence based clinical practice and professional development

Contribute to the development of local policies in relation to the implementation of national and regional strategies

Practice within the Legal & Ethical framework as established by Nursing Midwifery Council (NMC)

Performance is monitored on an ongoing basis and is appraised annually.

Education Practice Development

To participate in the clinical supervision framework for nurses within Spirit Healthcare Ltd.

To offer appropriate advice to nurses and other professions on care practices, delivery and service development within the parameters of the role and scope of practice.

To participate in the ongoing individual professional development within the organisation

To access the mandatory training programme.

Maintain professional practice through continuing education, professional updating and when/where available, involvement with professional specialist groups.

Supports with teaching and mentoring of undergraduate nursing students, graduate nurse practitioner students, and other members of the healthcare team as appropriate (eg physiotherapy, radiology, FY2 medical staff).

Actively promote and disseminate research-based practice.

Audit current practice and negotiate appropriate changes to practice.

Participate in relevant research / audit projects to service and practice needs and in line with nursing research strategy

Provide support for staff undertaking research, audit or teaching activities.

Organisational/Managerial:

Organise own time in line with agreed job plan.

Accountable for the effective management of resources including stores supplies pharmacy and maintenance of equipment.

Contribute to strategic issues relating to the nurse practitioner service and the specialty.

Act in a collaborative and advisory role to share knowledge and expertise to help deliver high standards of patient care.

Promote the development of a flexible and skilled nursing workforce.
