We're recruiting for a new part-time role within our adult mental health service, focused on providing practical support to patients throughout their recovery journey. From helping individuals set achievable goals to accompanying them to appointments and fostering community connections, you'll play a vital role in empowering patients and strengthening local support networks. If you have experience as a Health Coach or Social Prescribing Link Worker (Mental Health), and you're passionate about improving mental health outcomes, we'd love to hear from you.

Main duties of the job

We are excited to introduce a new part-time role designed to strengthen support within the PCNs adult mental health service.

The purpose of this position is to provide practical, hands-on assistance to patients as they progress through their treatment. The role will include helping individuals set achievable goals, supporting them in following through with identified activities or referrals, and offering motivational coaching to maintain positivity and engagement. The role may involve accompanying patients to appointments or community activities and fostering connections between patients, GP practices, and local services to enhance community integration. Working closely with our Mental Health Practitioner this role will play a key part in ensuring patients feel supported and empowered throughout their recovery.

As this is a new position, we are seeking candidates with either a Health Coaching background or experience as a Mental Health Social Prescribing Link Worker, bringing the right mix of skills and enthusiasm to make a meaningful impact. We are looking for someone to support, encourage, and motivate patients in improving their mental health and overall wellbeing through empowering, person-centred interventions.

The successful post holder will provide personalised, holistic support to patients and will:

Coach and motivate patients through multiple sessions to help them identify their strengths, set meaningful goals, and take active steps towards increased social engagement and community involvement.

Deliver interventions such as self-management education, peer support, and group sessions for patients and families where appropriate.

Facilitate access to support services, including financial and benefit advice, housing, health and safety home systems, and leisure/social activities. This may involve accompanying patients to appointments and groups.

Make referrals to local community groups and other agencies, sharing information and identifying gaps in provision.

Assist patients in building resilience and making informed decisions about their health and wellbeing.

Work in partnership with GPs, Mental Health Practitioners, and the wider multidisciplinary team to connect patients to community-based activities and services that enable them to take increased control of their health and wellbeing.

Develop networks and partnerships with other agencies, attend multi-disciplinary meetings, and participate in local events to raise awareness of the service.
