West Kent Primary Care is looking to recruit an Assistant Operations Manager on behalf of Tunbridge Wells Primary Care Network (PCN).Tunbridge Wells Primary Care Network
(PCN) is made up of 8 member GP practices servicing over 84,000 patients. We are a dynamic and cohesive PCN offering
services including First Contact MSK Assessment, Social Prescribing, Paramedics
and Clinical Pharmacy.
Main duties of the job
The Assistant Operations
Manager and Lead Care Coordinator will support the PCN Business & Strategy Manager
in managing and developing services available to patients across Tunbridge
Wells. The overarching goal is to
support the work of the Primary Care Network enabling, promoting, and developing
the effective use of data, information, knowledge and technology to improve,
inform and support the delivery of Tunbridge Wells PCN contracts and services
for its patients.

Job responsibilities
The post holder will work closely with the other Assistant Operations
Manager to ensure the smooth running of the PCN including:
-
Building and maintaining PCN clinics on Emis
- Research and development of new PCN services as
well as a lead role in the operational implementation of these services including;
MSK assessment, mental health projects, etc. bringing quantifiable benefit to
patients across these areas
- Liaising with PCN Social Prescribers utilising population health intelligence to
proactively identify and work with a cohort of patients to deliver personalised
care
- Directing practices to maximise referral to
social prescribing link workers to ensure patients are supported to take up
training and employment, and to access appropriate benefits where eligible
- Managing
Care Coordinators to provide coordination and navigation for patients and their
carers across health and care services with particular focus on; care homes new
patient admissions, management of student placements
within the PCN, development and management of the PCN website and PCN digital
support.

- Previous experience as a practice or medical receptionist
- Knowledge of administrative procedures and information analysis
- Experience of minute taking

- Able to use Microsoft Office applications with above average capability with Word, Excel and PowerPoint.
