Job summary
We
have a new and exciting opportunity to appoint Housebound Healthcare Assistants
to join our dynamic and friendly operations team. Post holders will be
confident in dealing with patients, able to work independently whilst understanding the importance of proactively
escalating concerns.
Main duties of the job
You will be providing the delivery of
services including Phlebotomy, 24hr
BP monitoring, ECGs, Warfarin Monitoring, Atrial Fibrillation Monitoring to some of the most vulnerable
patients in Hillingdon in their homes. Ideally you will already be skilled in
providing phlebotomy services and should be willing to be trained to support
the delivery of other aspects of patient care.
You will be supported in your
role through friendly management and administration team as well as clinical
management for all patient concerns. Hillingdon celebrates working in

our outstanding training hub providing support and development to clinicians
working in the area.
This will require you to loan work
and travel between clinical sites and patients homes within, and up to 1 miles
outside of the borough of Hillingdon.

Job responsibilities

In accordance with the services
needs the post-holder will make him/her-self available to undertake a variety
of duties including Phlebotomy,
ECG, AF monitoring, Post-operative dressings, Pill
Checks, Immunisations, Phlebotomy, Spirometry, BP checks, NHS Health Checks and
diabetic foot checks according to the agreed and provided processes and
protocols. Training will be provided in accordance with business requirements.

Assessing the health needs of
patients with chronic health needs and using your own judgment to assess
whether further input is needed.

Ensuring that potential
safeguarding concerns are reported immediately

Screening patients for disease risk
factors and early signs of illness.

Contributing to QOF requirements
and recording data appropriately.

Contributing to PCN requirements
and recording data appropriately.

In consultation with patients and
in line with current practice disease management protocols, developing care
plans for health related illnesses.

Screening patients for health
education.

Admitting and discharging patients
to and from the caseload and referring to other care providers as appropriate.

Recording clear and contemporaneous
consultation notes to agreed standards.

Collecting data for audit purposes
and contributing to the practice audit programme as required.

example: coordinating aspects of governance, QOF, enhanced services, CPD and
