We have an exciting opportunity for a full-time Advanced Clinical Practitioner (ACP) to join our well-established multidisciplinary team. You will work across all of our clinical sites in Haverhill, Long Melford, Lavenham and Glemsford, as well as our centralised Hub in Haverhill where clinical triage and remote consultations are undertaken.

Main duties of the job

  • Deliver high-quality, patient-centred clinical care, including assessment, diagnosis, treatment and management of a wide range of acute and chronic conditions
  • Work autonomously within your scope of practice, including independent prescribing
  • Manage a mix of face-to-face, telephone and digital consultations, including triaged Hub-based work
  • Support the management of same-day demand and urgent care presentations
  • Undertake clinical triage using the eConsult system
  • Work collaboratively as part of a multidisciplinary team, including GPs, pharmacists, nurses, ECPs, PAs
  • Contribute to clinical governance, audit and service improvement activities
  • Support and mentor junior staff where appropriate, contributing to a positive learning environment

Suffolk GP Federation CIC is a not-for-profit community interest company. Our main purpose is to support Primary Care across Suffolk including support with working collaboratively and at scale. We also deliver a portfolio of NHS services across Suffolk and North East Essex, including Podiatry, Ultrasound, Diabetes, as well as urgent Care Services including the Out of Hours face-to-face Service in Suffolk.

Our team includes GPs, Clinical Pharmacists, Pharmacy Technicians, Nurses, Care Coordinators and other allied health professionals, all working together to provide high-quality, accessible care.

  • Demonstrate strong clinical assessment and decision-making skills
  • Have excellent communication and interpersonal skills
  • Are adaptable and able to work across multiple sites and consultation types
  • Are motivated, proactive and committed to delivering high-quality patient care
  • Work well both independently and as part of a team
  • Are confident using, or willing to learn, new IT and digital consultation systems
