Job summary
We are seeking a Diabetes Specialist Nurse to join our team on a part time, permanent basis. The role involves managing complex diabetic cases, providing education and promoting self-management to improve patient outcomes. Working closely with the wider primary care team, the Diabetic Specialist Nurse will ensure that high-quality, evidence-based care is delivered, following best practices in diabetes management.
This is an excellent opportunity for a local Practice Nurse who has a specialist interest in Diabetes Management and is committed to providing high-quality care in a primary care setting.
Main duties of the job
- You'll need to be a registered nurse, with an interest in Diabetes
- You'd be suited to this role if you have a caring and empathetic nature
- Its important that you can work well as part of a team as well as independently
- You'll need to be able to pick up new IT systems with supportive training

- Postgraduate certification or diploma in Diabetes Care
- Extensive experience in managing diabetes care, including complex cases such as insulin initiating and adjustment
- Strong understanding of diabetes pathophysiology, treatment options, and lifestyle interventions for managing the condition
- Excellent communication skills to educate and support patients, families and other healthcare professionals
- Ability to work independently and as part of a multidisciplinary team
- Experience in primary care or community settings.
Desirable
- Mentor/ teaching qualification.
- Experience of teaching or mentoring
- Independent and/or supplementary nurse prescribing qualification
