Leicester, Leicestershire and Rutland Integrated Care Board are looking for an enthusiastic and pro-active clinician to join the Personalisation team. As a Personalised Care Clinician, you will be part of the Clinical Quality and Personalisation directorate at the heart of improving patient care.

This role will be pivotal in influencing the delivery of high-quality care within social care placements and community settings in LLR, working closely with ICB colleagues from the Clinical Quality & Personalisation Directorate and wider.You will be supporting and monitoring independent providers, including care homes, domiciliary care providers and independent hospitals. Successful candidates will be able to build strong relationships with providers alongside our partner stakeholders, including the 3 Local Authorities, Commissioning Support Unit and CQC.

The successful candidate will be highly motivated and eager to work in challenging environments and across various settings. A car driver is essential due to travelling to different locations.

Main duties of the job

Supporting a number of ICB priorities and key objectives you will work collaboratively with partner agencies. This work will include:

Routine visits to monitor and support quality assurance of providers, including additional escalation support for providers when required. To ensure they are managing, delivering, and sustaining the required quality of care as determined by their contract.

Collect information to assure the standards of care being provided and promote good practice and to respond appropriately to any poor standards of care, training needs or areas of concern, that may be identified and work with colleagues to address these.

Support the targeting and eligibility of resources, monitoring, implementation, and evaluation of the tasks/projects by providing high quality support including complex information, analysis and communication.

Be a key member of the team as well as supporting effective communication and stakeholder management, both internally and externally.
