Job summary

NCL ICB is looking to recruit a Senior Complex Care Practitioner, who will support the Integrated Care Board (ICB) in driving transformation as well as value for money in planning, commissioning and delivery of Complex Individualised Care for our clients with mental health needs, learning disability or for children and young people.

The role is crucial to the operational delivery of the Complex Care priorities and will evolve over time as the Directorate matures and becomes embedded within the Integrated Care System.

The post holder will act as an expert senior Learning Disability nurse providing specialist clinical and care management advice and support for adults with a Learning Disability diagnosis who have complex needs and are being considered for or eligible for Continuing Healthcare, and acting as a point of escalation for complex cases managed by a team of LD Complex Care Practitioners.

Please note that priority will be given to NCL ICB employees who are affected by change.

Main duties of the job

The post holder will be managed by the Clinical Team Manager and will have formal links with area and hospital social work teams, hospital discharge teams, Integrated learning Disabilities teams and community healthcare teams. The post holder will also regularly liaise with local health agencies and the voluntary/independent sector, residential/nursing homes and domiciliary care providers, working with those with a Learning Disability diagnosis.

The post holder will also be responsible for liaising with social work colleagues and Learning Disability teams regarding clients who have joint health and social care needs and who require specialist health input, and when necessary, to be actively involved in developing care plans and advising on appropriate packages of care.

The post holder will work in close partnership with Local Authorities, NHS colleagues and care providers. We are therefore looking for someone who has experience of building strong working relationships with suppliers/providers, hence good communication and inter-personal skills are crucial.

The post holder will be influential and contribute to standardisation and change in conjunction with wider programmes and projects within the directorate, as required.

The post holder will line manage complex care practitioners as required within the emerging Complex Care Directorate structures.

  • Equivalent level of experience or educated to masters level.
  • Qualified Registered LD Nurse, with current registration with NMC
  • Must have an understanding of the background to and aims of current healthcare policy related to complex individualised commissioning and appreciate the implications of this on engagement with stakeholders and delivery of services
