Job summary

Following a re-structure of our ways of working a fantastic opportunity has arisen for proficient and highly motivated registered clinicians (Nurse or AHP) to join our existing Continuing Healthcare team. This role require a dynamic individual who is an effective communicator, is forward thinking and able to perform both independently and as part of a team.

The ideal candidate will have the ability to co-ordinate and manage competing priorities, good organisational skills, time management and a solid understanding of the National Framework for NHS Continuing Healthcare and NHS funded Nursing Care.

Main duties of the job

Your daily working environment will offer varied and interesting aspects of NHS services. Continuing Healthcare is an assessed funding stream and therefore you will get the chance to interact with various stake holders, patients, and their families on a regular basis. If you are able to work proficiently, flexibly, accurately and able to meet the challenge then this is the job for you.

Qualifications

Essential

  • Clinically qualified for example -RMN/RNMH/RGN. Evidence of continuing professional development.

Desirable

  • Experience or further study in dementia care, project management

Knowledge

Essential

  • Experience within NHS or Local Authority. Knowledge of continuing care policy and procedures to include: Eligibility, FNC, appeals process and the national framework of CHC. Minimum of three years' experience in relevant clinical practice Knowledge of MHA, Consent to treatment, DoLS, Transforming Care and children's legislation. Knowledge of Social Services eligibility systems and procedures. Experience of interpretation of national policy and implementation. Development and maintenance of good working relationships across partner organisations. Knowledge of advocacy services

  • Understanding of the wider health and social care agenda, system transformation, service integration Experience of financial budgets

Physical Skills

Essential

  • Basic keyboard skills. Able to drive and hold a clean UK driving licence and be insured for business use

  • Flexible and able to work as a member of the team. Politically astute. Resilient. Self-confidence. Personal drive. Empathy. Excellent planning and organisation skills.
