Job summary

Within our All Age Continuing Care service we undertake assessments for NHS funding and case management in 2 dedicated teams.

This post holder will manage all of the case management functions for individuals on CHC, Joint Funding , Section 117, and FNC in Shropshire Telford and Wrekin ICB locality .

This is currently done by managing 7 individual teams providing case management in the following areas , Mental Health , Learning Disabilities , Care in the persons own home , care in Nursing homes and for those in receipt of a PHB. most of these teams are led by a Band 7 and supported by 1 or 2 Band 6 case managers. However, in the future we may move to locality areas .

Main duties of the job

? To have expert subject knowledge in relation to a wide variety of Individual Commissioning functions,including, but not limited to NHS Continuing Healthcare, NHS Funded Nursing Care, relevant sectionsof the Mental Health Act, NHS Act 2006 (as revised), Who Pays and responsible commissionerguidance. To use this knowledge to advise members of the public, team members and wider staff withinand outside the organisation.

NHS Shropshire Telford and Wrekin was created on 1 July 2022 and is responsible for planning and buying a wide range of health and care services for the whole of Shropshire Telford and Wrekin. These include GP and primary care services, hospital care, community healthcare and mental health services. We also have a duty to monitor these services to ensure they provide a high level of care and are value for money. We are clinically led and work closely with the 51 GP practices across the county. This means we can have closer links to our patients so we can develop more personalised local health services.
