Continuing Healthcare (CHC) Team Administrator
Monday - Friday, no shifts, no weekends or bank
holiday working.
Full training, peer support and supportive
management.
The post holder will often be required to work
using their own initiative and unsupervised within defined policies and
procedures and is expected to respond to day to day issues that arise and seek
management support as required; organising and prioritising own workload to set
deadlines.
This is an exciting opportunity to work within
a strong and supportive team and to undertake a key role in successfully
delivering important patient focused care services for NHS Humber and North
Yorkshire integrated care board.
This is a varied role, working in a busy team,
which requires someone with exceptional administration skills who can
multi-task effectively to provide comprehensive and efficient support to
colleagues, service providers and with the Local Authority
Main duties of the job
Working
under the instruction of the line manager to develop an understanding of the
service area and job role, the post holder will be responsible for providing
efficient and effective administrative support for the CHC which will involve:
Acting
as the first point of contact for the organisation, receiving, responding to
and effectively sign-posting incoming calls within the organisation;
Accurate
recording in clinical and administrative electronic records used within the CHC
Team;
Consistently
delivering a customer focused service; endeavouring to meet the needs of those
accessing the CHC team in a timely manner, achieving stated deadlines;
Providing
secretarial and administrative services to the specific CHC teams as tasked,
which will include support for members of the public accessing the CHC team,
other professionals and service users; Develop a broad understanding of CHC process and programmes of work;
Responsible
for organising meetings as required

Job responsibilities
To
act as a first point of contact for the CHC and Complex Care Team, dealing with
routine and specialist enquiries and referring to others as required.
Provide
high quality administrative support to the functions of the CHC Team.
Communicating
sensitive information to families of clients with complex needs.
Provide
a customer relationship management service via the telephone, email, internet
or other contact channels
Produce
correspondence and communications ensuring accuracy and that they all are
produced in line all procedural documentation.
Receive,
transmit, record and retrieve information and manage, present and share data as
appropriate.
Updating
and checking patient information on CHC Systems and other relevant systems to
ensure accuracy of data.
Input
into and support reporting and monitoring functions, as required.
Contribute
to the development of local policies and procedures.
Liaison
with providers, local authorities, health colleagues and members of the public
regarding NHS CHC, FNC and Section 117.
Filing
in appropriate formats and ensuring suitable storage or distribution.
Receiving
incoming post, faxes and e-mails and distributing to the team
Drafting
and typing letters and reports to professionals and members of the public.
Act
as first point of call for all funding requests and responsible for ensuring
the correct information is received, scanned recorded and triaged to the
relevant member of staff following data protection polices
To
attend statutory training and keep up to date with personal developments.
To
use your own skills and experience to educate other professionals in the
department to develop a wide range of experience within the team.
To
maintain confidentiality at all times
To
carry out any other duties as needed by the department of behalf of the ICB
