As a Senior Community
Programme Manager at NHS Humber and North Yorkshire Integrated Care Board (HNY
ICB) working within the ICBs Community Health and Care Collaborative, you will
play a pivotal role in transforming community health and care services for our
population. This dynamic position requires a strategic thinker with extensive
experience in the health and care system, capable of leading and overseeing the
planning, delivery, and monitoring of various transformation initiatives. You
will support the Strategic Lead by working autonomously managing multiple
projects, collaborating with senior leaders and various teams across the system
to drive innovation and improve health outcomes for the HNY population.

Main duties of the job
You will be responsible for
supporting the Strategic Lead to delivery key priorities of transformation across
the ICB. You will lead the transformation of multiple projects within community
health and care services, working on delivering new models of care and linking
together agencies to integrate pathways and improve patient experiences for our
population. Your goal will be to improve health outcomes for the community by
driving innovation and reducing inequalities. There are two posts available,
one permanent, one fixed term which will have specific portfolios of work
reportable to the Strategic Lead.
Key skills and qualities
include strong leadership, excellent communication, and the ability to work in
a highly political and sensitive environment. You must be adept at managing
complex data, presenting reports at Board level, and building effective relationships
with stakeholders.

- Educated to masters level or equivalent level of experience of working at a senior level in specialist area.
- Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to masters level equivalent.
- Evidence of post qualifying and continuing professional development.
Desirable
- Prince 2 or programme management qualification or relevant equivalent experience
