An opportunity has arisen within the
ICB for colleagues who are qualified as RMN/RNLD clinical role to consider a
change in direction and join the admission avoidance team.
We are looking to recruit a dynamic
and innovate Band 7 admission avoidance manager to work as part of the team, chairing
care education and treatment reviews (CETR), ensuring that the person is at
the centre of their care and treatment.
The post holder will work with
individuals with learning disabilities and or autistic people and their
families, both within hospital settings and or within the community to seek
solutions that support people to remain living safe and well within their
communities and avoid admission to hospital.
The post holder will ensure that the
CETR is carried out in a manner that is based on the person-centred values
laid out in the national CETR policy.
We are able to offer an upskilling
period for those who apply but may not have all the required skills and
experience to begin the role immediately. This will likely take a maximum of
2 months Upskilling will include undertaking digital record training
alongside shadowing and support from existing team colleagues.
Main duties of the job
The post
holder will be employed by Humber and North Yorkshire ICB, as part of the admission
avoidance team, supporting the Learning Disability and Autism agenda
across all Places within the geography
(e.g. North Yorkshire and York and Humber)
They will
work closely with members of the ICB Case Managers and Local Authority Social
care managers. The post holder will lead
on the continued implementation of the revised national Care Education &
Treatment Review (CETR) Policy and Guidance (March 2017), undertaking community
and inpatient CETRs and providing specialised advice for people with learning
disabilities and/or autism who display behaviours which challenge under the
Transforming Care Partnership programme.
The
postholder will provide a chair function for both community and inpatient
C(E)TRs providing highly specialised advice for people with learning
disabilities and/or autism who display behaviours which challenge under the
Transforming Care Partnership (learning disabilities and austism) programme.
Working as
part of a Admission Avoidance Team, the postholder will work with other CTR and
CETR Lead colleagues to develop processes and working practices which will form
a cohesive approach to C(E)TRs, LAEPs and Dynamic Support Registers across

The
postholder will respond to requests to chair Local Area Emergency Protocol
(LAEP) meetings, ensure that any actions are followed up and use the outcome to
inform the Dynamic Support Register (DSR). As a result the postholder will need
to coordinate support and actions across the system to respond in a timely and
effective manner to prevent further escalation and crisis.
The post
holder will coordinate action planning with health, social care, education and
criminal
justice
professionals, and families following reviews to ensure that recommendations
and outcomes from a CETR are achieved. This will involve the discussion of
highly complex, sensitive and contentious information, where persuasion,
empathetic and re-assurance skills are required in order to gain agreement or
co-operation.
The post
holder will also provide data and information to meet national reporting
requirements and in support of the ICB Learning Disability and Autism Board.
The post
holder will be required to travel across the ICB place based areas, and the
North of England on a regular basis.
