The YHCR product and technology stack and associated governance are operated by Interweave, an organisation which is NHS owned, operated, and which is funded by a collaborative ICS approach.

The HNY Shared Care Record Delivery Team works closely with Interweave and is responsible for working with health and care organisations in HNY to enable them to get connected to the YHCR, both as providers of data into the shared care record and also enabling them to consume or view patient data for direct care purposes from other organisations. The work of the team is central in enabling HNY to realise the clinical, patient, efficiency, and cost-saving benefits of shared care records.
