Job summary
We are
seeking to recruit a motivated, enthusiastic and dedicated Registered Mental
Health Nurse, to work alongside the Case Management Continuing Healthcare Team as
a B7 Vulnerable Adults Clinical Team Leader.
They
be required to lead on developing the team and support case managers with
complex case management issues.
Working
autonomously and taking accountability for their own professional actions the
post holder will commission the care of vulnerable people including those with
mental illness, learning disabilities, and provide Case Management of their
clinical care and care pathway.
Provide
support to the review of out of contract placements and the development of risk sharing
arrangements with providers. Working across the geographical areas of North Yorkshire
and York and further afield.
Working
Monday to Friday, no shifts, no weekends or bank holidays, full training, peer
support and supportive management.

Main duties of the job
The post holder will be
required to work closely with Local Authorities and commissioned ICB services
to ensure the S117 pathway is adhered to. Ensuring a timely informed discharge from
hospital to placement.
Working closely with local
agencies to develop pathways in line with the Mental Health Act.
The post holder will re-assess
all new patient placements within initial 6-8 weeks assessment period to ensure
placement suitability and discuss with patient quality and care issues.
Manage highly complex,
sensitive and contentious patient information duringinteractions with legal
representatives and independent provider clinical teams where the environment can be hostile,
antagonistic or highly emotive.
The post holder will need to
use a high level of communication skill to ensure messages are understood where
there may be barriers to understanding and gain agreement and co-operations.
The post holder will be
required to meet with patients and carers within service and ensure clear
understanding of case management and complex programmes of care on a continuous
basis, discuss potential transfers to alternative units with patients when
necessary.
