Job summary
An exciting opportunity has arisen for an enthusiastic and experienced registered
Social Worker to join the Continuing Health Care Team, within East Riding place
as part of NHS Hull and North Yorkshire ICB
This is an integrated team compromising of Clinicians / Social Workers /
Legal Team, supported by a dedicated Admin Team.
As a band 6 Social Worker you will take responsibility as delegated by
the CHC Senior Team for the assessment and transformation of care for existing
and potential patients who are eligible for Continuing Healthcare / Section 117
aftercare.
In this role you will support the Senior Social Worker to case manage a
complex caseload of people with varied needs including but not limited to physical
disabilities, a wider range of health and social care needs, including mental
ill health and / or learning disabilities, who require ongoing care and
support.
There will be close working with residents, carers,colleagues, Local authorities, the acute trusts, the
independent sector, the wider ICB and any other relevant groups as part of the
wider MDT. This may include specialist learning disability and autism care
settings or placements within the NHS / independent sector
As a Case Manager you will promote best practice
and be responsible for providing expert professional advice and guidance to
colleagues, promoting excellent working relationships, liaising with colleagues
from other disciplines, both externally and internal to the ICB
Main duties of the job
Manage a caseload , with some complex cases
and chair case conferences where
appropriate
Undertake and support the decision making
where there are complex, high risk situations this will include supporting
social workers and clinicians with legislative dilemmas and complex mental
capacity/best interest decisions
Give guidance and expert advice on a daily basis
to qualified staff. Model good practice in the team and take a lead in
supporting the professional development of the team workforce. Supervise
clinicians and qualified social work staff, identifying their development needs
in line with the appraisal process
Within delegated authority agree a range of
support to meet the identified outcomes of eligible adults
To
promote and model the use of critically reflective practice
To
implement a joint training and development program to be delivered to health
and social care staff working in
hospitals and the community
Excellent communication and assessment skills are required as well as
being able to demonstrate an expert approach in relation to problem solving,
team management and workload prioritisation.

- Diploma level specialist knowledge required through training /experience
- Evidence of contribution to the development of training programmes / courses and delivering training
- Knowledge of continuous developments in NHS and Social Care
- Ability to understand the Continuing Care and Funded Nursing Care criteria and the ability to apply this to everyday practice
- Ability to update knowledge of various acts and statutory guidance such as the Mental Health Act, Mental Capacity Act, Safeguarding etc
- Knowledge of risk management and governance
Desirable
- Relevant courses which represent ongoing professional development
- Good working knowledge of local policies that apply to nursing, health and social care
- Knowledge of adult protection and other policies that affect vulnerable and chronically sick people
