NHS HNY ICB, East Riding of
Yorkshire Place is seeking to recruit a highly motivated, enthusiastic and
forward thinking registered mental health or learning disability nurse to work
as part of a professional team. The successful candidate will need to be able
to demonstrate knowledge and skills in person-centred assessments, and the
ability to work within a multi-agency environment.
You will work autonomously and
take accountability for your own professional actions reviewing care for vulnerable
people including those with mental illness, learning disabilities, and provide
Case Management of their clinical care and care pathway.

Main duties of the job
The post holder will be
required to work closely with Local Authorities and commissioned ICB services
to ensure the S117 pathway is adhered to. Ensuring a timely informed discharge
from hospital to placement.
Working closely with local
agencies to develop pathways in line with the Mental Health Act.
The post holder will re-assess
all new patient placements within initial 6-8 weeks assessment period to ensure
placement suitability and discuss with patient quality and care issues.
Manage highly complex,
sensitive and contentious patient information during interactions with legal
representatives and independent provider clinical teams where the environment
can be hostile, antagonistic or highly emotive.
The post holder will need to
use a high level of communication skill to ensure messages are understood where
there may be barriers to understanding and gain agreement and co-operations.
The post holder will be
required to meet with patients and carers within the service and ensure clear
understanding of case management and complex programmes of care on a continuous
basis, discuss potential transfers to alternative units with patients when
necessary.

The postholder will review case
manage patients with highly complex histories who are a risk of exhibiting
extreme and unpredictable levels of challenging behaviours. This will involve
providing specialist advice relating to their individual care package and
planning for future care provision.
Working in partnership with
East Riding of Yorkshire Council adult and children directorates review
bespoke packages of care for people with diagnosed mental health and/or
learning disabilities who reside within the Community. The post holder will
work closely with East Riding of Yorkshire Council colleagues to ensure there
is a co-ordinated approach to commissioning and case management of care which
results in robust and effective care packages.
Manage complex, sensitive, and
contentious patient information during interactions with legal representatives,
criminal justice and healthcare provider clinical teams. The post holder will
need to use a high level of communication skill to ensure messages are
understood where there may be barriers to understanding and gain agreement and
co-operation.
Engage with patients and
carers who receive bespoke packages of care to ensure clear understanding of
case management ensuring the individual patient is fully consulted and involved
in their care pathway and represent East Riding of Yorkshire Health & Care
Partnership at patient Care Programme Approach (CPA) reviews, Care (Education)
and Treatment Reviews (CETRs), Section 117 Reviews.
The postholder will request
patient review meetings when concerns are raised by the patient, their carer or
by other partner ICBs to discuss quality of care issues and care pathways, working
closely with East Riding of Yorkshire Council to identify alternative service
providers which offer effective safe service provision.

across multiple care providers across health and social care
To
have an understanding of the criminal justice system including MAPPA Level 2
and Level 3 - and be able to provide
expertise and advice on the interface of individuals who are in receipt of
custodial care and who have complex healthcare needs with associated risks to
themselves or the public. This is often
in relation to people who have committed a criminal offence and who are of high
interest to the media and public.
The postholder will organise
daily workload to respond to any urgent issues which may arise - to ensure the
patient and public are kept safe. Attend meetings with a range of agencies and
professionals, recommending options to ensure the patients care package is
safe and effective and delivers value for money.
Ensure good working
relationships are maintained with service users, the team and external
agencies, through effective and clear communication. Work collaboratively with
the Complex Case managers and CHC team to problem solve and find solutions
where concerns exist in relation to decision making around appropriate care for
service users ensuring effective use of NHS resources.
To demonstrate a collaborative
and influencing style of working, negotiating with others to achieve the best
outcomes and overcome barriers. Ensure all documentation is legible and written
in accordance with professional guidelines and stored in line with Information
