Job summary

Are you a Public Health Nurse or Paediatric Nurse with experience of working with Children and Young People, looking for a change and career development? Do you have the passion, expertise, and drive to make a difference in the lives of children and young people with complex health needs? Gloucestershire ICB is seeking a skilled and motivated person to join us as a Childrens and Young Peoples Continuing Care Nurse Assessor in our established, friendly, and fast-paced team. A comprehensive induction programme will be offered.

Main duties of the job

You will work collaboratively with multi-agency professionals, including Local Authority, Education, parents, families, and advocates, ensuring that care is equitable, holistic and timely. This role will involve offering Personal Health Budgets including Direct Payments, working with existing care providers and supporting in developing a Commissioned care provider service network.

You will be supported to develop expertise in the National Framework for Children and Young Peoples Continuing Care (CYPCC), helping children and families access equitable care and support to meet their complex needs.

The role will involve travel within Gloucestershire and on occasion out of County for face-to-face assessments.

Undertake comprehensive initial and review Children & Young Peoples Continuing Care (CYPCC) Assessments in line with the National Framework (2016)

Support CYPCC eligible individuals and or their representatives working collaboratively with families, carers and multi-disciplinary teams to design and implement personalised care packages.

Monitor and review care packages to ensure they remain responsive to changing needs.

Contribute to service development, quality improvement and safeguarding initiatives across Gloucestershire.

Support the development and review of CYPCC Personal Health Budgets (PHB) in line with the CYPCC National Framework (2016).

Ensure accurate and open communication and co-ordination with a range of organisations and individuals, research and draft correspondence and papers and ensuring the management of specific tasks, lead reporting and analysis across a range of specialties, functions and projects including contributing to policies, procedures and pathways and CCC local and National Frameworks.

Support in the strategic development and oversight of local transition planning processes and planning meetings. Undertaking CYPCC assessments and care planning to ensure smooth transition to adult services.

Be a key member of the team as well as supporting effective communication and stakeholder management, both internally and externally.
