Job summary

The Continuing Healthcare Team is looking to appoint a Case Worker to support the Continuing Healthcare Team Nurse Assessors in the review of care packages and ongoing case management of those individuals in receipt of health funding. This is an exciting new role within the Team which is a busy operational team within Gloucestershire ICB.

We would also welcome applications from Nursing Associates for this role.

Main duties of the job

The Case Worker team will work alongside the team of Nurse Assessors within a range of settings and will undertake the non-clinical case management of those in receipt of NHS Continuing Healthcare (CHC), Funded Nursing Care (FNC) and other sources of health funding. This will include liaising with placements, monitoring of care provision and changes to presentation and providing quality assurance. The Case Worker Team will support the clinical staff to ensure the care provision meets the needs of an individual, maintain accurate records, and support all aspects of work for those individuals in receipt of health funding.

The post holder will have experience of working within a range of different health and care settings. You will have excellent organisational skills and be able to work independently and creatively, using a problem-solving approach. It is essential that you are motivated, with strong communication skills, a team player and enjoy the challenge of working in a fast-paced environment. The successful candidate will need to be able to work well under pressure, managing competing demands and prioritise according to the requirements of the service. There is a requirement for the post-holder to hold a driving license and have the ability to travel throughout the county.

Essential

  • Educated to NVQ 3 level in a relevant subject or equivalent level of qualification or significant equivalent previous proven experience.
  • Significant administrative/ secretarial experience including initiating and maintaining office systems.
  • Demonstrable experience in dealing with the public and dealing with sensitive and confidential information.
  • Intermediate knowledge of IT systems and software programmes such as Outlook, MS Teams Word, Excel, PowerPoint and web based systems.
  • Understanding of Confidentiality and Data Protection Act.
  • Knowledge of NHS issues.

Desirable

  • Knowledge and experience of person centred approaches to care provision.
  • Awareness of current good practice in the care of people with mental health/dementia/challenging behavior.

Communication skills
