Job summary

An exciting opportunity has arisen within the Gloucestershire Continuing Healthcare (CHC) Team for a Complex Case Manager (Band 7) registered professional. The role is planned to build on a combination of Complex Case Management skills & subject matter expertise, and to develop and support a growing service and work autonomously within a supportive team.

In this role you will support the wider CHC team by case managing highly complex individuals and their families/carers helping them to access the most appropriate care to support their needs. The role can be challenging and therefore rewarding as well diverse day to day. The role includes case managing a variety of patients i.e. physical health, learning disability, mental health, ABI, etc. in a variety of settings.

You will work across the ICB footprint visiting patients in a variety of settings including their own homes as well as meetings on MS Teams. There will also be times where visits are required to out of county placements.

Main duties of the job

Day to day case management for CHC eligible patients with highly complex needs including monitoring high cost packages.

Making decisions around complex CHC cases in accordance with the CHC National Framework.

Court of Protection case management which includes attendance in court representing the ICB.

Working with care providers and social care in the best interest of the patient and to provide the best care possible.

Safeguarding highly complex and challenging individuals.

Working for the patient to achieve outcomes that are best for them.

Presenting cases to Risk & Scrutiny Funding Panel.

Joint work with partners within all commissioned workstreams (health & social Care).

The vacancy is for 37.5 hours a week, Monday Friday, 09.00 - 17.00 hrs.

The post holder will support the Head of Department and their team to ensure that their workstream of programmes are planned and managed effectively and take the lead in assisting in their successful delivery:

The post holder will work autonomously to support and co-ordinate, where necessary, the case management of individuals with highly complex health and social care needs. The post holder will be working to support, individuals and their families/carers in accessing the most appropriate care to meet their needs.

Responsible for providing day to day case management expertise for CHC eligible patients presenting with highly complex needs and circumstances including monitoring of high cost care packages and 1 - 1 provision.

Define current processes, facilitate discussions and advise colleagues regarding all matters concerning safeguarding, Court of Protection, mental capacity, spinal/brain injury pathways and best interest decisions.

Ensure the case management is delivered with the patient at the heart of the process and that case management issues are dealt with in line with best practice in a timely manner to achieve best outcomes for the individual.

The post holder will be responsible for overseeing the transfer of domiciliary packages to a Personal Healthcare Budgets and writing and reviewing all Support Plans and risk assessments.
