Nurse assessor and Personal Health Budget Case Worker

Job summary

We are building on our success at NHS Gloucestershire ICB and developing our Childrens Continuing Care team. An exciting opportunity has arisen to recruit to a new post, and we are looking for a motivated and enthusiastic nurse with experience in children's nursing, which may also include learning disabilities or mental health services. Childrens continuing care is the name given to a package of care that is arranged and funded by the NHS for children and young people who have an unmet health need that cannot be met by universal and specialist services.

The successful candidate will have excellent clinical knowledge as well as communication and assessment skills; be resilient and able to work autonomously but also work as part of a team. Good knowledge of safeguarding and multi- agency working is required. This exciting new post is a community-based post with the post holder expected to travel throughout Gloucestershire. Occasional out of County travel may also be required.

Main duties of the job

  • To work within the team to develop and oversee Personal Health Budgets (PHB) for patients who have been found Continuing Care (CC) eligible.

  • To implement the National Framework for Continuing Care (CC) in a timely, efficient, and transparent manner. This includes detailed assessments to identify the level of care needs, and to make recommendations for the commissioning of care.

  • To support the further development and implementation of Personal Health Budgets to the wider Continuing Care team.

  • To support Continuing Care and Personal Health Budget reviews of individuals in line with the National Framework.

  • To contribute to the ongoing development of Personal Health Budgets and local frameworks
