Main duties of the job
Our Continuing Healthcare (CHC) Team is seeking an NMC Registered Nurse (Band 6) to join our busy CHC Team, which is part of the wider Integrated Commissioning Directorate.
The post holder will be responsible for facilitating multi-disciplinary CHC assessments and patient reviews in line with The National Framework (July 2022 Revised) and local standard operating procedures.
This role will require undertaking a breadth of CHC assessments and come to eligibility decisions on patients (which will include regular face to face assessments across Gloucestershire).
Our team is looking for a professional with excellent assessment and interpersonal skills and with a passion for supporting individuals with complex needs. Experience in the field of CHC is desirable, but not essential.
Good organisational skills with a sound knowledge of MS Office packages, alongside excellent written and verbal communication skills is also required.
Training and support will be available to the right candidate.

Essential
- RN/ RMN 1st level
- Up to date NMC registration
- Relevant Degree/evidence of being able to work at that level.
- At least three years registered nurse experience.
- Working knowledge of the National Framework for CHC and FNC.
- Working knowledge of Primary Care Services.
- Palliative Care experience.
- Evidence of up-to-date clinical skills.
- Experience of caseload management.
- To be able to manage a caseload of patients with complex needs.
- Ability to undertake complex nursing assessments.
- Able to monitor high standards of care.
- Able to direct and co-ordinate a programme of work, collaboratively or autonomously.
- Knowledge of legal issues regarding NHS funding.
- Fully aware of NICE guidelines for the provision of specialist equipment and Long Term Conditions.
- Knowledge of promotion of continence.
- IT experience.
- Highly developed communication skills, both verbal and written, including the writing of comprehensive reports.
- Evidence of continuing professional development at degree level.
- Confidence to work alone.
- Team player.
- Adaptable and flexible.
- Calm under pressure.
- Enthusiasm for the speciality.
- Able to foster collaborative working relationships.
