Job summary

NHS Continuing Healthcare (NHS CHC) is a package of care for adults aged 18 or over which is arranged and funded solely by the NHS. North West London Integrated Commissioning Board (ICB) are looking for individuals to join the service in a variety of roles and bands. The NWL team has come together under a new structure and this is an exciting opportunity to join a dynamic and positive team.

North West London Integrated Care Board is looking to appoint one full time Learning Disabilities Case Managers. The post holder will report to the CHC Complex Clinical Lead. The post holder is required to have a sound track record of working with adults with Learning Disabilities and or Autism and manage the LD caseload and ensure correct processes are followed in delivery of the National Framework for NHS Continuing Healthcare & NHS Funded Nursing Care.

The post holder will:

Be responsible for leading on the key operational day-to-day areas of work relating to CHC for people. Ensuring that the CHC service is fully compliant with and following the national frameworks. Support joint working with the LA ICBs and clinical teams within North West London

As an experienced nurse or AHP you will have a working knowledge of Mental Capacity Act, Safeguarding, experience of undertaking assessments, case and risk management and personalised care agenda will be essential for this role

You will complete all aspects of NHS Continuing Healthcare and strive to improve processes and services.

Main duties of the job

We are looking for innovative, forward thinking, enthusiastic and confident nurses or AHPs to be part of the skilled and dynamic team. Previous experience of working in NHS Continuing Healthcare would be an advantage and a level of knowledge and experience of the framework is required.

The post-holder will undertake various functions relating to the National Framework for NHS Continuing Healthcare which will include undertaking health needs assessments, facilitation of multi-disciplinary assessments, reviews, supporting individuals with case management and staff leadership and management.

The Post Holder will act as an advocate for individuals with learning disabilities, ensuring their needs and preferences are considered throughout the CHC process. This may involve supporting them in accessing appropriate healthcare services, coordinating with external service providers, and empowering them to make informed decisions about their care

You will be an excellent communicator with sound interpersonal skills. You will need to have the ability to manage emotive and sensitive issues with tact and diplomacy, work well under pressure, negotiate with confidence, and maintain a high level of productivity. You will become part of a diverse multi-skilled team, who support each other with their own clinical knowledge to ensure the best personalised care for each individual client.

Work closely with a variety of stakeholders, including healthcare professionals, social workers, care providers, and specialist learning disabilities teams. Collaboration is essential to ensure effective communication, coordination, and integration of care services. Actively participate in multidisciplinary meetings, case conferences, and care reviews.

Individuals with learning disabilities may have specific risks related to their care and support needs. identify, assess, and mitigate these risks to ensure their safety and well-being. This may involve implementing appropriate safeguarding measures, coordinating with relevant professionals, and providing education and support to caregivers.

Liaise with finance colleagues and ensure that placement costs are accurately reflected in service budget

Information Management

Accurate and timely documentation is vital in managing and monitoring individuals' CHC cases. Document assessment findings, care plans, progress notes, and any changes in the individual's health or care needs. This documentation ensures transparency, continuity of care, and adherence to legal and regulatory requirements.

Planning and Organisation

Regularly monitor the quality and effectiveness of CHC services provided to individuals with learning disabilities. This includes conducting periodic reviews of the care plan, assessing outcomes, and identifying areas for improvement. collaborate with the individual, their family, and the care team to adjust the care plan as needed and ensure it remains person-cantered.

Policy and Service Development

Actively contribute to the continuous improvement of CHC services for individuals with learning disabilities. This may involve participating in quality improvement initiatives, sharing best practices, and offering feedback to senior management regarding challenges and opportunities for improvement within the CHC system.

Ensure best practices, through the use of research, and policies related to supporting individuals with learning disabilities.
