NHS Continuing Healthcare (NHS CHC) is a package of care for adults aged 18 or over which is arranged and funded solely by the NHS. North West London Integrated Commissioning Board (ICB) are looking for individuals to join the service in a variety of roles and bands. The NWL team has come together under a new structure and this is an exciting opportunity to join a dynamic and positive team.
North West London Integrated Care Board is looking to appoint a full time CHC Complex Clinical Assessor. The post holder will report to the CHC Complex Clinical Case Manager.
The post holder will be responsible for providing day to day case management expertise for CHC eligible patients presenting with highly complex needs and circumstances including supporting complex safeguarding situations, providing support to the Head of CHC with legal cases, such as court of protection.
Main duties of the job
We are looking for innovative, forward thinking, enthusiastic and confident nurses to be part of a skilled and dynamic team. Previous experience of working in NHS Continuing Healthcare would be an advantage and a sound level of knowledge and experience of the National Framework for NHS Continuing Healthcare is required.
The post holder will need to be an excellent communicator with sound interpersonal skills. They will need to have the ability to manage emotive and sensitive issues with tact and diplomacy, work well under pressure, negotiate with confidence, and maintain a high level of productivity. They will become part of a diverse multi-skilled team, who support each other with their own clinical knowledge to ensure the best personalised care for each individual client.The post holder will complete all aspects of NHS Continuing Healthcare and strive to improve processes and services. They will respond and report safeguarding concerns working closely with system partners. They will ensure that DoLS compliance is followed. They will need to demonstrate time management skills in managing a caseload, and working in a flexible manner within a pressurized environment.
