Job summary
Norfolk
and Suffolk ICB are offering opportunities for Registered Nurses (Adult, Mental
Health or Learning Disability) to join the NHS Continuing Healthcare (CHC)
team. These roles involve coordinating CHC assessments, case managing
individuals eligible for CHC funding, and supporting the Discharge to Assess
model. The team is supportive and forward-thinking, seeking motivated nurses
keen to develop skills in a dynamic environment.
The
roles include undertaking CHC assessments in homes, care homes or virtually,
supporting personalised care through Personal Health Budgets, and commissioning
and managing care packages or placements. Staff work across Norfolk and Suffolk
with bases in Ipswich and Norwich, collaborating with local authorities,
hospitals, community services, care providers and voluntary organisations to
improve patient experience.
These
rewarding roles offer opportunities to rotate within the team to build
knowledge and expertise. A full induction and training programme is provided,
along with ongoing professional development. Previous CHC experience is not
essential, but applicants should have experience assessing individuals with
complex needs and coordinating care.
Candidates must be
strong communicators with good IT skills and able to manage sensitive
situations professionally. A driving licence and access to a car are required.
Full and part-time roles are available, minimum 22.5 hours per week.
Main duties of the job
Responsible for coordinating and carrying out comprehensive assessments and making eligibility
recommendations using the appropriate local and national tools as defined in the guidance for NHS
CHC and NHS-FNC.
To collate data and evidence from comprehensive assessments to form reports.
Responsible for completion and review of person-centred, outcome-focussed care and support plans
for individuals deemed eligible for NHS funding, ensuring that the agreed package of care and
support meet the individuals assessed needs.
To support individuals to access Personal Health Budgets to manage their care and support.
To carry out reviews of care and support and eligibility for NHS funding where required.
To support with the local resolution process where eligibility decisions have been challenged.
To carry out Mental Capacity Assessments and make Best Interests Decisions as appropriate, in line
with relevant legislation.
To support with and be involved in the process of making applications to the Court of Protection or
Responsible Body for authorisation of Deprivation of Liberty, including consideration of the necessity
and proportionality of proposed deprivations.
