Job summary

Suffolk and North East Essex Integrated Care Board are looking for an enthusiastic, skilled professional to support our continued system ambition of reducing the reliance on inpatient care for adult people with a Learning Disability and/ or autistic people. The role of the case manager will support on system planning, developing the dynamic support register and implementing care, education and treatment reviews.

Please note, due to the ongoing cost reduction programme and restructure underway at SNEE ICB (due to end in Summer 2024), prior consideration for this role may be given to existing staff who are affected by this programme and this role may be removed from the recruitment process at any time.

Main duties of the job

  • To impart highly developed specialist knowledge, underpinned by theory and experience to system partners on working with people with a learning disability and autistic people.
  • To undertake in-depth patient safety and quality reviews for people in our communities in specialist mental health inpatient facilities and act as host commissioner.
  • To work closely with the provider collaborative on specialised commissioning for Suffolk patients.
  • To undertake assessments interpreting complex needs of patients or client conditions, appropriate diagnosis; for overseeing specialised treatment programmes for those who on the Dynamic Support Register.
  • To use expert investigative and analytical clinical reasoning skills and evidence-based knowledge to interpret highly complex facts, to make accurate analysis of clients health and social care needs.
  • To use advanced theoretical and practical knowledge of complex needs and assessment findings to recommend an integrated health/social care plan from a range of options available and to refer to other agencies as necessary.
  • To work to standards within the Mental Capacity Act, Mental Health Act, Safeguarding or deprivation of liberty standards (DOLs).
  • To work with legal professionals to submit any reports required as part of the Mental Health Act and DOLs and attend any legal meetings suggested by council.
