Job summary
Suffolk and
North East Essex ICB have an exciting opportunity for a 12-month fixed term
Commissioning Placement Officer to work within our NHS Continuing Healthcare
Team.
The role will
be part of a dedicated Commissioning Team whose
purpose is commissioning care for people with varied physical, emotional and
mental health needs, who are eligible for NHS Continuing Healthcare
funding. This includes care for people
at the end of their lives.
The
post holder will directly commission care for people including those living in
their own home and care home placements under the guidance of clinicians.
The role will involve having a proactive and compassionate approach and
engaging directly with people with health and care needs. You will be required
to liaise with care providers, local acute hospitals, community services and
voluntary organisations daily.
The
role will require the successful individual to have excellent communication and
organisational skills, to be resilient, work at pace, and thrive under
pressure. It also requires good negotiation skills, accurate record-keeping
skills, and an agile response to deadlines.
Please note, due to the ongoing cost reduction programme and restructure underway at SNEE ICB (due to end in Summer 2024), prior consideration for this role may be given to existing staff who are affected by this programme and this role may be removed from the recruitment process at any time.
Main duties of the job
To
ensure that all commissioning requests are actioned within agreed timeframes.
To
source placements/packages of care according to individual needs and current ICB
policies including negotiation of costs directly with providers
To
ensure the efficient use of contracted and approved providers.
To
monitor market sectors, providing the necessary information and evidence to
enable clinicians to maintain market intelligence.
Negotiate
spot purchases for a care package where the need cannot be met through the
usual contracts.
Ensure
that all financial approval processes are adhered to in accordance with the
Financial Approval Panel terms of reference, as required to meet the ICB
financial governance and audit measures.
To
ensure that Service Level Agreements (SLA) are completed for all commissioned
care packages/placements.
Ensure
that all patient related actions are recorded in the patient management
information system in a timely manner.
Ensure
that the most up-to-date registers and databases of providers are used.
Identify
and respond to urgent or critical situations referred, by liaising with clinicians
and providers and finding a solution appropriate to the needs of the patient
Support
clinicians by undertaking duties under guidance including attendance at daily
telephone conference calls with system partners to support patient flow.

To work in collaboration with clinicians, to review, verify and arrange Placements, Support Services and Support Packages that deliver the specified requirements for those eligible for NHS Continuing Healthcare funding. Ensure that all requests for a service or support package identified by clinicians within the NHS Continuing Healthcare team are placed adhering to service specification standards.
The Administrator will provide a full and comprehensive administrative support to the NHS Continuing Healthcare (CHC) team, in the implementation of the National Framework to support the CHC nursing team to enable them to carry out their work effectively and all aspects of their respective roles within the ICB.
The role requires a highly motivated and enthusiastic individual to provide administrative services including the organisation and co-ordination of meetings, minute taking and disseminating information to a range of internal and external contacts and taking appropriate follow-up action as required.
The ability to manage emotive and sensitive issues and conversations with tact and diplomacy is essential as at times both patients and carers may be distressed. Managing difficult conversations can be both challenging and rewarding in our person-centered service requiring an understanding and compassion for people at a difficult time in their lives.
You will need to have demonstrable IT skills, including Microsoft Excel, Word, Teams and Outlook and will receive in-house training on how to use a clinical system for recording patient information.
The successful candidate will be part of a team where the focus is on continuous improvement to ensure we deliver high standards and work productively to deliver quality, timely services to our patients.
You will be supported by a dynamic team of commissioners led by experienced nurses.
