Job summary

Are you a senior doctor, nurse, AHP, pharmacist, psychologist, optometrist, social worker, dentist or any other registered health or care professional who wants to make a real difference to patients and their outcomes? We are looking for six excellent people for the following roles:

Cancer (0.4 wte)

Children, Young People and Maternity (0.4 wte)

End of Life and Frailty (0.4 wte)

Inclusion, Inequalities and Long Term Conditions (0.2 wte)

Mental Health (0.4 wte)

Urgent and Emergency Care (UEC) and Elective (0.4 wte)

We have a track record of Can Do Health and Care and have created these new posts to actively find people to build on the great work being delivered and bring new ideas and different ways of achieving the best for our patients.

Main duties of the job

Create and maintain communities of practice amongst relevant health and care professionals

Leadership within an integrated team in the ICB to deliver excellent performance outcomes

Leadership within an integrated team in the ICB on development of relevant strategy, clinical pathways and policy

Leadership of relevant research and innovation

Leadership within medicines management workstreams

Contribute to the leadership and delivery of the ICB objectives

Personal development to increase effectiveness and set or maintain career trajectory

The APD for Inclusion, Inequalities and Long Term Conditions will work as part of a wider multi-disciplinary team across the ICB and wider ICS to achieve the Stay Well outcome: Supporting adults with health or care concerns to access support and maintain healthy, productive and fulfilling lives, and the consequent priorities to achieve this outcome as outlined with the SNEE Joint Forward Plan (2023-2028). The ICB focusses upon the following LTC areas of work, although the postholder would be invited to explore new areas of work dependent on variation in outcome and aligned to stakeholder priorities:

Aligned to the Inequalities Committee, the postholder will:

Work with clinicians and professionals to build knowledge, including equipping colleagues with health inequalities knowledge and how it links to their specific role.

Be mindful of the impact of the social determinants of health and how they affect access, and their role in patient experience in patient-facing encounters given the impact of peoples experience on their outcomes.

Influence policy across the system to ensure we have the right conditions to tackle health inequalities.

End of Life and Frailty

Programme SRO: Judi Newman (CEO St Elizabeths Hospice, Chair SNEE End of Life Committee, judi.newman@stelizabethhospice.org.uk); Alliance Directors

The APD for End of Life and Frailty will work as part of a wider multi-disciplinary team across the ICB and wider ICS to achieve the Age Well and Die Well outcomes: Supporting people to live safely and independently as they grow older and Giving individuals nearing end of life choice around their care, and the consequent priorities to achieve this outcome as outlined with the SNEE Joint Forward Plan (2023-2028).

The SNEE End of Life workstream is delivered at place as a system-wide approach, committed to the six National Ambitions for Palliative and End of Life Care promoting patient-centred coordinated care. The priority for the End of Life Group is to improve early identification of those in the last year of life, enabling an increase in advance care planning and the associated improvements in care outcomes. The workstream includes the improvement of bereavement services and encouraging greater cultural confidence in talking about death and dying through the Compassionate Communities movement.

Frailty improvement is also delivered across at place in each of our Alliances, and work is aligned to the Age Well domain. Whilst separate from End of Life care, the post holder will optimise the links between end of life and frailty workstreams to facilitate seamless pathways for people in their last years of life.

Urgent and Emergency Care (UEC) and Elective

Programme SRO: Paul Gibara (Director of Performance Improvement, paul.gibara@snee.nhs.uk)

The APD for UEC and Elective will work as part of a wider multi-disciplinary team across the ICB and wider ICS to achieve the Stay Well outcomes: Supporting adults with health or care concerns to access support and maintain healthy, productive and fulfilling lives, and the consequent priorities to achieve this outcome as outlined with the SNEE Joint Forward Plan (2023-2028). Key priorities for the UEC and elective programmes include:

Improving access to diagnostics

Planned surgery and treatment when people need it, in the place of their choice

Best experience of planned care, reducing health inequalities and preventing clinical harm

People are signposted to the most appropriate service for their needs every time

People in need receive timely emergency care aligned with best practice

People with urgent and minor ailments/illnesses are managed outside of the ED

Strengthen and improve discharge processes

Mental Health

Programme Director: Eugene Staunton (Deputy Director of Transformation, eugene.staunton@snee.nhs.uk)

The APD for Mental Health will work as part of a wider multi-disciplinary team across the ICB and wider ICS to achieve the Feel Well outcome: Supporting the mental wellbeing of our population and the consequent priorities to achieve this outcome as outlined with the SNEE Joint Forward Plan (2023-2028). The post will focus upon the following areas of work:

Community Adults Mental Health (including Serious Mental Illness and Talking Therapies)

Mental Health Crisis (including all aspects of our all age crisis model)

Specialist Mental Health (including perinatal services, eating disorders, early intervention in psychosis)
