Job responsibilities

Role purpose

NEE SPECIFIC

Support Alliance initiatives around the prescribing of appliances by reviewing patients on behalf of the practices i.e., formulary adherence and appliance use reviews (AURs).

Support and provide clinical expertise to practices who have wound carer as their individualised target as part of the yearly medicines optimisation plans

Support prescribing support technicians with switches relating to appliances.

Training ICS wide primary care staff in appliance related prescribing i.e., practice teams, community pharmacists, care homes

To offer clinical expertise and guidance to the Alliance and its system partners and providers.

To Attend relevant meetings on behalf of the alliance linking with the wider Essex

To review all the contracts , routes and pathways for the provision of appliances with the Alliance e.g., the Online ordering system (ONPOS) and advice on their improvement and/or redesign.

Deal with all appliances related queries from clinicians and the Patient Advice and liaison service (PALS)

To support SNEE ICBs commitment to commissioning services that are both safe and effective in meeting the needs of the local population, ensuring that pathways in which appliances are used deliver cost-effective use of resources and improved patient outcomes and experience.

To support system wide integration, working with wider health and care professionals to ensure that use of appliances in health and care delivery settings is joined up.

To support the implementation of appliance related strategies across the NEE, IES and WS Alliances to:

o improve health outcomes;

o reduce avoidable harm;

o avoid waste.

o improve cost effective use of appliances

To improve the health of the population by optimising the use of appliances under the guidance of the senior appliance nurses

o promoting the safe, evidence-based and cost-effective use of appliances;

o providing up to date, unbiased information about treatments and care pathways;

o supporting practitioners and patients to make the best use of appliances;

o developing local guidelines and care pathways to optimise the use of appliances;

o supporting other health and care providers to deliver medicines optimisation in a way that is high quality, efficient, safe, well led, timely and responsive, effective and equitable.

Accountability

The post-holder will work within organisational policy and professional/ethical guidelines and without supervision.

The post-holder will identify opportunities for development and improvement across all clinical areas relating to the prescribing and/or use of appliances in primary care settings - this will include (but is not limited to) wound care, continence and stoma - and will support with implementing change under the direction and support of the senior appliance nurses.

The post-holder will act with a high level of independence, identifying the need for, developing, and initiating programmes of work aligned to their own portfolio and/or organisational priorities. Their work will be self-directed but supported by the senior appliance nurses.

Qualifications

Essential

• Professional knowledge acquired through degree in adult nursing.
• Registered with the NMC as a nurse.
• Evidence of continuing professional development.

Desirable

• Additional specialist knowledge to masters level (in the relevant area) acquired via formal training and/or other forms of training and/or experience

Experience

Essential

• Significant post registration experience.
• Experience of working in health or social care.
• Experience of using critical appraisal skills to aid decision making.
• Experience of planning, developing, and implementing change impacting beyond own area of work.
• Experience of delivering contentious information in a hostile environment.
• Experience of influencing a wide range of stakeholders.
• Experience of training/presenting.
• Experience of analysing complex data.

Desirable

• Experience of working in a GP practice.
• Experience of working with care homes.
• Experience of writing reports for clinicians and/or non-clinical staff.

• Experience in using sources of prescribing data e.g. ePACT2, Open Prescribing.

Knowledge

Essential

• Highly developed specialist knowledge in all clinical areas relating to the prescribing/use of appliances in primary care.
• Knowledge of the NHS structure and commissioning arrangements.
• Knowledge and understanding of the challenges facing primary and secondary care.

Desirable

• Non-medical prescribing qualification knowledge acquired only. This role does not entail prescribing.
• Knowledge of CQC and inspection process
• Knowledge of the Network Contract DES and IIF metrics.
• Knowledge of the social care sector and funding mechanisms to include care homes and home care services.
• Knowledge of community pharmacy funding mechanisms as they relate to appliances.
• Thorough knowledge of NICE guidance SC1

Skills

Essential

• Able to use a keyboard to enter clinical, prescribing or financial data with a very high degree of accuracy.
• Able to apply highly developed specialist knowledge when undertaking the review of patient records and complex prescribing patterns.
• Able to document clinical information and recommendations accurately using nationally approved coding.
