We have a vacancy within our service & are looking for a highly motivated, enthusiastic professional to join our supportive & dynamic team.

The role involves, completing robust & holistic health care assessments, leading multi-disciplinary team meetings, care planning safe and effective packages of care & supporting individuals with complex care needs to maintain their quality of life. This role regularly involves providing support & guidance to both our clients, their families, & external professionals, in order to achieve the best outcomes.

The team is responsible for upholding the guidance provided by the National Framework for Children's &Young Peoples & therefore, comprehensive training is provided to ensure this specialist knowledge is gained.

Main duties of the job

We are looking for a registered professional who is experienced in working with children & young people who have complex physical & mental health needs.

Successful candidates will have excellent communication & assessment skills; be resilient, able to work autonomously but also work as part of a team. This is a home-based role, & post holders will be expected to be able to travel across Cambridgeshire & Peterborough & out of area as required.

You will be joining a fast-paced team that employs the use of holistic nursing, inquisitive thinking & a focus on high quality. The nature of the role is autonomous & challenging but with the benefit of supportive colleagues.

About us

NHS Cambridgeshire & Peterborough is an Integrated Care Board (ICB) and the statutory NHS organisation responsible for planning and delivering local health and care services to the population and communities of Cambridgeshire & Peterborough.

  • Ability to produce any documentation to a high standard

Desirable
