Job summary
Continuing Care is the name given to a package of care that is arranged and funded by the NHS for children and young people who have an unmet health need that cannot be met by universal and specialist services.
We are currently looking for a motivated and enthusiastic children's nurse with case management experience who has excellent clinical knowledge as well as communication and assessment skills. The applicant should be resilient and able to work autonomously but also work as part of a team. Good knowledge of safeguarding, learning disability and mental health conditions and multi-agency working is required.
Main duties of the job
An understanding of the continuing care framwork is required alongside experience of case management and multi-agency working is an essential requirement for the post as close collaboration takes place with community and acute services as well as education and social care.
This is a community-based post and although the role is based in Birmingham and Solihull, the post holder will be expected to be able to travel covering the whole of the area and some out of area travelling may be required. We are currently continuing to work from home to support children and families.
The role will be varied with the practitioner undertaking assessments, attending MDTs and delivering training to multi agency professionals
