Job summary

The All Age Continuing Health Care (AACHC) team are seeking experienced, proactive and passionate Registered Nurses with current knowledge and recent experience of completing CHC Assessments and CHC Case Management, to join our dynamic, friendly team. This is a rare opportunity to work autonomously and collaboratively in a multi-agency environment, within a fast paced team. The primary focus of the role is Case Management, supporting individuals and their representatives through all stages of the CHC Appeals Process.

As a Senior CHC Appeals Nurse you will be responsible for implementing the NHS England CHC Framework for Appeals of CHC funding decisions where patients have not been awarded full CHC funding for their care. This includes the robust review of the previous MDT assessments and decision making processes followed during the assessment process for Continuing Healthcare Eligibility.

This is an excellent opportunity for an experienced CHC Nurse Practitioner to gain a wide range of experience working with individuals who have a broad range of health care needs and to work in a service that is constantly committed to improving patient care and experience.

Main duties of the job

As a Senior CHC Nurse Assessor in the AACC Appeals Team, you will be

  • using your CHC knowledge and a wide range of your clinical skills to ensure that outcomes are determined in a consistent manner to ensure effective application and interpretation of the National Framework for NHS CHC (2022).
  • reviewing and progressing eligible applications for review of Periods of Previously Unassessed Care (PUPoC) and
  • ensuringrobust eligibility recommendations can be made using the NHS CHC Framework process for completing retrospective Decision Support Tool assessments.
  • proactively case manage a caseload comprising of both Current informal and formal Local Appealand PUPoC cases, including collating and compiling evidence in detailed review document and maintaining robust, accurate, contemporaneous records within digital databases.
  • working collaboratively with all key stakeholders and colleagues within the NHS BSOL AACC Team to review CHC appeals and CHC PUPoC requests to ensure that the NHS Continuing Healthcare Framework and Best Practice Guidance andprocesses have been adhered to.
  • Providing additionalknowledge and expertise, when required,to support clinical and non-clinical colleagues reviewing issues arising from both Appeals and Previously Unassessed and Assessed Periods of Care (PUPoC).
