We have an exciting opportunity for an experienced, compassionate and skilled Enhanced Care Review Nurse to join our UEC team, providing excellent clinical leadership to the UEC team whilst supporting improvement and transformation of these services across the Birmingham and Solihull integrated care system.
This role focuses on improving patient discharge pathways and case management, particularly for people with complex dementia who may also have 1:1 nursing requirements, to ensure a safe, person-centred transition from hospital to independent sector D2A placements.
As an Enhanced Care Review Nurse, you will lead face-to-face reviews of patients receiving 1:1 care within Discharge to Assess (D2A) settings, working closely with a MDT professionals. You will help to shape safe and appropriate discharge decisions, reduce unnecessary prolonged stays, and explore alternatives to intensive care packages through collaborative working and innovation.
Main duties of the job
You will be responsible for reviewing and case managing the discharge journey of patients. In particular those with complex dementia who are placed in independent sector D2A beds with 1:1 support, ensuring clinical appropriateness and promoting timely, safe discharge.
You will coordinate multidisciplinary input, assess ongoing clinical need, and support the reduction of 1:1 care where appropriate by exploring alternatives such as assistive technologies and community support.
You will collaborate with key stakeholders including the CHC and discharge teams, hospital discharge hubs, local authority colleagues, mental health services, and care providers across the Birmingham and Solihull system. You will educate and improve the referral process for this cohort of patients, in particular around 1-1 care requirements and onward timely flow from these beds, and support the transformation and improvement of the delivery of these services with the selected stakeholders.
You will need strong assessment, clinical judgement, and negotiation skills, alongside a deep understanding of discharge legislation, the Mental Capacity Act, and safeguarding frameworks.
