As a diabetes pharmacist, you will be joining an award-winning and innovative team helping to implement the ICS' strategy of improving care for people with diabetes. Your responsibilities will include conducting targeted medication reviews to support better clinical outcomes and cost-effective prescribing for people with diabetes, contributing to diabetes multi-disciplinary team meetings in GP practices, undertaking clinical audits and project work, developing local diabetes guidance, responding to queries from other healthcare professionals and promoting medicines safety. You will develop and maintain key relationships with allocated GP practices, as well as diabetes specialists from secondary care and community teams.

As a member of the MMO Team, patients will be at the heart of everything you do - you will be joining a team dedicated to delivering high quality care and tackling health inequalities.
