Job summary
The Transition Nurse is a brand-new post which has been developed within the CHC Learning Disability Team. It is an exciting opportunity to develop a pathway to ensure the smooth transition of care for young people who require transition from the children's Continuing Care into the adult pathway for CHC.
The successful candidate will provide a dedicated point of contact between clinicians, clients, and carers. We are currently looking for a motivated and enthusiastic nurse with experience in leading on transition.
Continuing Health Care is the name given to a package of care that is arranged and funded by the NHS for adults who have a primary health need that cannot be met by universal and specialist services.
The role will entail coordinating the process and pathways for young people transitioning to adult continuing health care and other adult health and social care provision, identifying gaps in the coordination of care and liaising with children and adult services to promote a seamless local multidisciplinary approach to transition.
Successful candidates will have excellent communication and assessment skills; be resilient, able to work autonomously but also work as part of a team. Excellent knowledge of safeguarding and multi-agency working is required.
Main duties of the job

Experience of multi-agency working is an essential requirement for the post as close collaboration takes place with community and acute services as well as education and social care. Working knowledge of the continuing healthcare framework and SEND agenda is required. Knowledge and understanding in the planning and delivery of the liberty protection is essential.
