Job summary

NHS Continuing Healthcare (CHC) is a package of continuing care that has been arranged and funded solely by the NHS to meet a person's physical and mental health needs arising from disability, accident or illness. It is for vulnerable people with complex, intense and unpredictable needs or people who are rapidly deteriorating and possibly entering a terminal phase.

NHS Frimley ICB's East Berkshire Continuing Healthcare Service is seeking to recruit two highly motivated, very well organised and experienced registered nurses with qualifications in LD, MH or General nursing or an Allied professional i.e. Physiotherapist, Occupational Therapist, Social Worker to join the team of CHC assessors to support in the processing of Continuing Healthcare and Funded Nursing Care assessments, reviews and case management.

A current driving licence and vehicle used for business use will be required.

Main duties of the job

You will have knowledge of the National Framework for Continuing Healthcare (Oct 2018) and be able to:

o Coordinate multi-disciplinary meetings with a range of professionals i.e. social care, care homes, other providers etc.

o Undertake high quality holistic assessments of patients, clearly identifying needs using assessment tools as required.

o Face to face reviews/assessments will be required.

o Manage a caseload of complex cases.

o Cover as Duty Nurse within the Continuing Healthcare Service.

You will be a key member of the team and will need to demonstrate skills in IT, report writing, managing complex patients. You will also have excellent verbal and written communication and have the ability to work in stressful situations and be able to prioritise workloads to meet deadlines. You will be working in partnership with our local authorities and experience of multi-disciplinary working is essential.

Work as part of a multi-disciplinary team, acting as a resource to provide nursing advice in relation to appropriate placement and management of clients.

Provide accurate advice and information to all regarding recommendations following completion of assessment process.

To manage a generic caseload of clients following their placements for Continuing Healthcare and function as a Duty Nurse within the Continuing Healthcare Team. These duties will require the postholder to work with other organisations.

Support teams to ensure that the portfolio of tasks/projects is planned, managed and delivered effectively.

Support and inform the targeting of resources, monitoring, implementation and evaluation of the tasks/projects by providing high quality support including complex information and analysis, communications and stakeholder management.

Ensure accurate and open communication and co-ordination with a range of organisations and individuals, researching and drafting correspondence and papers and ensuring the management of specific tasks, lead reporting and analysis across a range of specialities, functions and projects.

  • Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area.
  • Further training or significant experience in project management, financial management or supporting change management processes.
