Job summary

NHS Continuing Healthcare (CHC) is a package of continuing care that has been arranged and funded solely by the NHS to meet a person's physical and mental health needs arising from disability, accident or illness. It is for vulnerable people with complex, intense and unpredictable needs or people who are rapidly deteriorating and possibly entering a terminal phase.

NHS Frimley ICB's East Berkshire Continuing Healthcare Service is seeking to recruit a highly motivated, very well organised and experienced Registered Nurse with qualifications in LD, MH or General Nursing or an Allied professional i.e. Physiotherapist, Occupational Therapist, Social Worker to join the team of CHC assessors to support in the processing of Continuing Healthcare and Funded Nursing Care assessments, reviews and case management.

A current driving licence and vehicle used for business use will be required.

Main duties of the job

You will have knowledge of the National Framework for Continuing Healthcare (July 2022) and be able to:

Support and Management of Staff.

Day to day overseeing of the operations of business as usual within CHC Team.

Participating in an Independent Review Panels at NHSE.

Ratification of CHC assessments.

Liaising and participating in Non agreed Panels with Local Authority Partners.

Verifying Decision Support Tools completed by Nurse Assessors.

Case Management and reviews.

Face to face reviews/assessments will be required.

Fast Track clinical administration.

Participating in Local Resolution Meetings.

Support and care planning.

To manage a defined caseload for those CHC clients with complex health needs.

To oversee the delegation of workload within the team.

Oversee and audit Procurement Forms for high-cost packages before submission to High Risk Panel.

Cover as Duty Manager within the Continuing Healthcare Service.

This role also carries a Clinical Caseload of complex cases.

You will be a key member of the team and will need to demonstrate skills in IT, report writing, managing complex patients. You will also have excellent verbal and written communication and have the ability to work in stressful situations and be able to prioritise workloads to meet deadlines. You will be working in partnership with our local authorities and experience of multi-disciplinary working is essential.

Contribute to performance improvement, taking a lead for identified areas where agreed, and overall day to day management of the workstream performance.

To oversee the delegation of workload within the team.

To ensure that you and the clinical team adhere at all times to the relevant National Framework and guidance, NMC Code of Professional Conduct and work within established policies, procedures, protocols and systems ensuring quality standards and performance as directed by the Head of CHC Service Delivery and Placement Governance.

Provide coordination of and participate in relevant internal and external working groups.

Support the management of a risk and issues tracking mechanism and its proactive resolution and escalation processes.

Take a lead in the coordination of training, clinical consistency meetings, development and recruitment activity across the team.

Manage staff, undertaking appraisals and ensuring that appropriate training is delivered to staff.

Ensure that all complaints, comments and suggestions are dealt with appropriately, in accordance with policy, resolving where possible and escalating to line manager as appropriate.

To ensure own and teams compliance with all statutory and mandatory training.

To progress any disciplinary or capability issues, as necessary.

Contribute to improvements of current management information, analysing, reporting and suggesting procedures to enhance decision making processes.

Ensure timely and accurate information analysis and reporting to management on agreed areas of work.

Work with members of the team to investigate the causes of any variance from target/plan and proactively contribute to the implementation of solutions.

To ensure correct Continuing Healthcare guidance and process is followed and that services are delivered to maximise patient safety.

To comply with the Data Protection Act 1998 and Code of Practice on confidentiality and data protection.
